Dear Evan:
Please find the subject groundwater monitoring report attached. This includes the sampling and reporting
requirements of the above referenced permit, Part V, Section B 1 as well as the reporting requirements of
the overall Tank Farm Remedial Action Plan. Enclosed are: a) Form GW-59 for injection well CIW7 and
monitor well MWTF1CR, b) Form GW-59 for each of the fourteen withdrawal wells, c) piezometer level
monitoring field sheet, and d) the summary operational and maintenance report.
As discussed with you by phone, we have contacted the vendor that supplied PCS Phosphate with UV
treatment equipment and the replacement filters. We are currently evaluating a proposal from this vendor
to improve the operation of this equipment. I'll continue to keep you informed on the progress regarding
this system.

QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
February 2005 — April 2005
The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Ultraviolet Light Disinfection system
during the period.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The withdrawal water well pumps are monitored daily by the area Maintenance Department and
preventative maintenance work is performed as needed. The total monthly and quarterly withdrawal water

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
The following actions were performed on all fourteen pumps.
The pumps were flushed on February 1, 8, 14, 21, March 2, 11, 17, 22, April 1, 13, 18 and 25. The lines
were flushed on February 2 and March 3. On April 13 changed the compressor oil, separator and filters.
