QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
August 2000 — October 2000
The Tank Farm Remediation System consists of a series of seven (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Anthracite Sand Filter / Ultraviolet Light
(ASF/UV) disinfection system throughout the period.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The Withdrawal Well System was down for a total of 3.0 hours during the period. The system was down
on 10-25-00 for 3.0 hours to repair the air compressor. The withdrawal water well pumps are monitored
daily by the area Maintenance Department and preventative maintenance work is performed as needed.
The total monthly and quarterly withdrawal water volumes for the period are:

Some addrtionaI short penods of planned downtime for preventative maintenance,—pipe-headercleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
Confined Sand Formation Withdrawal Well (SWW) Pumps
SWW-1: On 8/14 replaced totalizer. On 8/17 the pump was shaken to start. On 9/1 the pump was
replaced.
SWW-2: On 8/17 & 8/22 the pump was shaken to start. On 10/11 the pump was replaced.
SWW-3: On 9/19 the airline was found plugged this was repaired. On 10/2 the pump was shaken to start.
SWW-4: On 10/11 the pump was shaken to start. On 10/13 the counter was replaced. On 10/31 the pump
was replaced.
SWW-5: On 10/20 the pump was shaken to start.
SWW-6: On 10/18 the pump was replaced.
S W W-7: Pump operated without incident.

Croatan Formation Withdrawal Well (CWW) Pumps
CWW-1: On 10/25 the pump was replaced.
CWW-2: On 8/31 & 9/26 the pump was shaken to start. On 10/5 the pump was acid washed.
CWW-3: On 8/14 the pump was replaced.
CWW-4: Pump operated without incident.
CWW-5: Pump operated without incident.
CWW-6: Pump operated without incident.
CWW-7: On 10/12 the air -regulating valve was replaced.
