Dear Evan:
Please find the subject groundwater monitoring report attached. This includes the sampling and reporting
requirements of the above referenced permit, Part V, Section B 1 as well as the reporting requirements of
the overall Tank Farm Remedial Action Plan. Enclosed are: a) Form GW-59 for injection well CIW7 and
monitor well MWTF1CR, b) Form GW-59 for each of the fourteen withdrawal wells, c) Form GW-59 for
monitor wells MWTF1A and MWTFIB, d) Form GW-59 for the WLCT, e) piezometer level monitoring
field sheet, and f) the summary operational and maintenance report.
The flow data and downtime information presented is from August, September and October. The
withdrawal data was collected in October. The injection well data was collected in November and
December. The report to be issued in February 2005 will represent data from November, December and
January.

PCS PHOSPHATE.- TANK FARM REMEDIAL ACTION PLAN
OUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
August 2004 — October 2004
The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Ultraviolet Light Disinfection system until
October 13, 2004 when the injection well system was shutdown to replace the filters and Ultraviolet Light
disinfection system. The new equipment installation was completed and brought online on October 24,
2004. The system operated with frequent trips, resulting in the replacement of the controller for the UV
System. Troubleshooting and optimization of the system continues with assistance from Culligan Water
Conditioning. Preliminary sampling was conducted at the Water Level Control Tank and CIW — 7. Both
samples were sampled for total coliform. The sample results were 260 and 41 (mf), 100 Mls respectively.
The following steps were taken to troubleshoot, facilitate sterilization and prevent contamination of the
injection water.
1. Install sample tap valve on the discharge line from the UV columns.
2. Down stream of sample tap install a means to inject NaOCI into the discharge line using a metering
pump.
3. Seal the Water level control tank outside the building by having all of the bolts put in place. Currently
there are only 4 bolts securing the flange.
4. Replace the screen on the 1" air vent on top of the water level control tank.
A total of 15 gallons of 12% NaOC1 was metered into the system down stream of the UV system. Samples
were taken down stream of the UV prior to NaOC1 injection to insure the performance of the UV system.
Additional sampling was conducted at the Water Level Control Tank and CIW — 7 on December 28, 2004.
Both samples were sampled for total coliform. The sample results were 5 and 390 (mf), 100 Mls
respectively. The source of high coliform in CIW — 7 was suspected to be the result of a failure of the well
header system. The system was inspected and no comprise in the header was located. It is theorized that
the source of the coliform is groundwater entering the well when the injection well system trips or is
shutdown. Additional testing was performed on January 26, 2005 which supports this theory. The sample
results for the Water Level Control Tank and CIW — 7 were 2 and 12 (mf), 100 Mls respectively. This data
reflects improved performance of the injection well system. Additional testing to determine the source of
the coliform is in process.

Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs

Withdrawal Well System
The withdrawal water well pumps are monitored daily by the area Maintenance Department and
preventative maintenance work is performed as needed. The total monthly and quarterly withdrawal water

The flows for November are Sand Formation 359,390 gallons, Croatan Formation 107,463 gallons. The
total flow is 466,853 gallons or 10.8 GPM.
Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
