Box 6 Murfreesboro, North Carolina 27855 Subject: Permit No. 7814 Town of Murfreesboro Non -Discharge Type Spray Irrigation Wastewater Treatment Facilities EPA Project No. C370491-02 Dear Mayor Hill: In accordance with your application received August 13, 1982, we are forwarding herewith Permit No. 7814, dated December 29, 1982, to the Town of Murfreesboro for the construction and operation of the subject non -discharge type wastewater treatment facilities. This Permit shall be effective from the date of issuance until December 29, 1987, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this Permit are unacceptable to you, you have the right to an adjudicatory hearing before a hearing officer upon written demand to the Director within thirty (30) days following receipt of this Permit, identifying the specific issues to be contended. Unless such demand is made, this Permit shall be final and binding. You are cautioned not to proceed with construction until the Authori- zation to Award Package, EEO and MBE documentation/certification has been reviewed and you are in receipt of our approval. One (I) set of final approved plans and specifications is to be for- warded to you, one (1) set retained for the Environmental Protection Agency (EPA), and two (2) sets forwarded to the Corps of Engineers. DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT Raleigh P E R M I T For the Discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules and Regulations PERMISSION IS HEREBY GRANTED TO Town of Murfreesboro Hertford County FOR THE construction and operation of 1,234 linear feet of 12—inch gravity sewers, and modifications to the existing Town of Murfreesboro Wastewater Treatment Facilities to increase the capacity to 0.476 MCD and to change the facility from a direct discharge type facility to a non —discharge land application type facility by modification of flow through the two existing wastewater ® treatment lagoons from parallel operation to series operation and an increase in total storage capacity to approximately 35 days at the design flow, and addition of a mechanical and manual bar screen, dual manually cleaned grit chambers, a Parshall flume with automatic flow recording, dual 825 GPM influent screw pumps, post chlorination, dual 1200 GPM spray irrigation pumps, approximately 7,600 linear feet of 12--inch discharge force main, spray irrigation laterals, approximately 77 acres of wetted spray irrigation area, five (5) groundwater monitoring wells, and a stand—by power generator to serve the Town of Murfreesboro with no discharge of wastewater to the surface waters of the State, pursuant to the application received August 13, 1982, and in conformity with the project plans, specifications, and other supporting data, subse— quently filed and approved by the Department of Natural Resources and Community Development and considered a part of this Permit. This Permit shall be effective from the date of issuance until December 29, 1987, and shall be subject to the following specified condi— tions and limitations: •1) This Permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifica— tions and other supporting data. 2) This Permit is effective only with respect to the nature and • volume of wastes described in the application and other support- ing data. � Permit No. 7814 Town of Murfreesboro December 29, 1982 Pg#2 3) The wastewater facilities shall be properly maintained and operated at all times. 4) This Permit shall become voidable.in the event of failure of the soil to adequately absorb the wastes, and may be rescinded unless the facilities are installed, maintained and operated in a manner which will protect the assigned water quality standards of the surface waters, and prevent any contamination of the ground waters which will render them unsatisfactory for normal use. 5) In the event the facilities fail to perform satisfactorily, the Permittee shall take such immediate corrective action as may be required by this Department. 6) If any nuisance condition arises concerning this land application/ spray irrigation facility, including the creation of an odor problem, the Permittee shall take such immediate corrective action as may be required by the Division. 7) Solids, sludges, or other pollutants removed or resulting from the wastewater treatment process shall be contained and disposed of in such a manner as to prevent any contamination of the surface or ground waters of the State. 0 8) Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. 9) The application rate shall not exceed 1.84 inches/acre/week. 10) In the event it is necessary to remove solids from the waste stabilization lagoon, the appropriate Permit must be obtained from the Division of Environmental Management. 11) This Permit may be rescinded if a suitable year round vegetative cover is not properly maintained and managed. 12) No root or leaf crops for direct human consumption shall be raised on the irrigation site. 13) Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. 14) Freeboard in the lagoons shall not be less than 1.5 feet at any time. 15) No other type of wastewater than described in the application and other supporting data shall be disposed of on this site. 11 • A Permit No. 7814 Town of Murfreesboro December 29, 1982 Pg#3 16) The Permittee shall employ a State certified wastewater treat- ment plant operator to be in responsible charge of the waste- water treatment facilities. Such an operator must hold a valid State certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. 17) The issuance of this Permit does not relieve the Town of Murfreesboro of the responsibility for contamination of surface or ground water resulting from the operation of this facility. 18) Construction of the sewers shall be so scheduled so as not to interrupt service by the existing utilities nor result in an overflow or bypass discharge of wastewater to the surface water of the State. 19) Monitoring wells shall be constructed as shown on the approved facilities plans and the following monitoring schedule imple- mented: Parameter *Frequency Sample Type Location Flow Daily Continuous Influent BOD5 Semi. -annually Grab Influent, wells COD Semi-annually Grab Influent, wells Total Kjeldahl N Semi-annually Grab Influent, wells NH3 as N Semi-annually Grab Influent, wells NO2 + NO3 Semi-annually Grab Influent, wells Total Phosphorus Semi-annually Grab Influent, wells Fecal Coliform bacteria Semi-annually Grab wells Water ravel Monthly wells *Semi-annual samples should be taken during late winter and late summer. The data shall be submitted to the Division of Environmental Management, Washington Regional Office inonthly. Each report shall be submitted within 45 days following the end of the reporting period. One set of samples must be taken before spraying begins. 20) Additional monitoring facilities and/or sampling and reporting shall be constructed and/or.implemented if deemed necessary by the Division of Environmental Management. 21) A minimum separation .from the spray irrigation wetted area shall be maintained as follows: aa. 50 feet from any public road right-of-way- b. 50 feet from any free flowing stream. C. 25 feet from any wet weather ditch. d. 100 feet from anylpotable well. Permit No. 7814 Town of Murfreesboro ® December 29, 1982 Pg#4 22) Adequate facilities shall be provided so as to prevent any surface water runoff from entering the spray field or carrying any land applied waste material into the surface waters of the State. 23) The Permittee, at least six (6) months prior to the expiration of this Permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if indicated, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. 24) During the construction of the proposed additions/modifications, the Permittee shall continue to properly maintain and operate the existing wastewater treatment facilities at all times, and in such manner, as necessary to comply with the effluent limits specified in the existing NPDES Permit. 25) Before either one of the existing treatment lagoons'is tempora-rily taken out of service as required by the approved project construction sequence, an Authorization to By-pass a portion of the wastewater treatment facilities must be obtained from the North Carolina Division of Environmental Management.
