Dear Ms. Joyner: In accordance with the application received on November 22, 2010 and additional information received March 4, 2011, we are forwarding permit number WI0700199. This permit is to inject Oxygen (ISOC) to remediate groundwater contaminated with gasoline and related compounds at the facility referenced above. This permit shall be effective from the date of issuance until February 28, 2013, and shall be subject to the conditions aid limitations stated therein, including the requirement to submit a final project evaluation as stated in PART VII — MONITORING AND REPORTING REQUIREMENTS. Please read the entire permit to ensure that you are aware of all compliance requirements of the permit. - You will need to notify this office by telephone 48 hours prior to initiation of operation of the facility. In order to continue uninterrupted legal use of the injection facility for the stated purpose, you must submit an application to renew the permit 120 days prior to its expiration date. Campbell) Subject: FW: Midway Motors -Bertie County iSOC application Mr. McCray, GMA's response to your request for clarification for the Midway Motors-Bertie County iSOC UIC permit application is summarized below: 1. Part C. of the application (page 4 of 7) indicates that the screened interval of the wells to be used is 5 feet to 30 feet and corresponding grout depths range from..3 feet to 23 feet. 1 do not have a schematic for the iSOC components, nor do 1 have the information for the grout depths on each individual RW that are being considered for iSOC installation, but having only seven feet of screened interval extending past the grout interval seems counter- productive. Please provide the grout depths or effective screen intervals for each well intended to be used. The iSOC injection wells (RW1, RW3, RW4, and RW5) are each screened from 5 to 30 feet below land surface (BLS), with the exception of well RW5 which is screened from 5 to 28 feet BLS. As you mentioned, the grout intervals listed for these wells was from 3 to 23 feet BLS in Part C (page 4 of 7) of the UIC permit application. However, this grout interval (3-23 feet BLS) is the average grout interval for all the monitoring wells at the site (see Table 1 in part G). Many of the monitoring downgradient of the source area are screened in the deeper portions of the surficial aquifer and within the underlying Yorktown aquifer to monitor the fate and transport of the contaminants originating from the source area. The iSOC injection wells (RW1, RW3, RW4, and RW5) are each grouted from land surface to approximately 3 feet BLS. The grout for each of these wells consists of cement from land surface to approximately 2 feet BLS, and bentonite from 1 2 to 3 feet BLS. The annulus space fr 3 feet BLS to the bottom of each well is filled with gravel pack. Therefore the effective screen intervals for each of these injection wells (RW1, RW3, RW4, and RW5) is from 5 feet BLS to the bottom of each well (either 28 or 30 feet BLS). 2. Also provide a schematic drawing of the iSOC components. A generalized copy of the design schematics for an iSOC system and its components obtained from the manufacturer are attached. 3. Page 5 in Part D. Injection Rationale, you state that "biodegradation has occurred mostly under aerobic or iron reducing conditions. There does not appear to be sufficient nitrate or sulfate to support anaerobic degredation." Does that mean that biodegradation is occurring in two separate RedOx zones of the aquifer, an aerobic zone, and an anaerobic zone in which Iron is reduced? Or does it mean something else, because I am unclear how Iron is reduced under aerobic conditions. As you had mentioned, biodegradation is occurring in two separate reduction -oxidation zones within the surficial aquifer at the site. Anaerobic biodegradation primarily by iron -reduction is occurring within the contaminant plume while aerobic degradation is occurring along the edges of the contaminant plume. This is evidenced by the spatial distribution and concentrations of DO, ORP, alkalinity, and dissolved iron across the monitoring -well network at the site (Table 2 in part G). 4. Also, can you provide a value for Transmissivity through the zone in which you are adding the DO? As described on page 2 part B (Hydrogeologic description) an 15ft/day is the estimated hydraulic conductivity of the surficial aquifer at the site. However, on February 21, 1996, a constant rate aquifer test that lasted approximately 1435 minutes was conducted utilizing monitoring well MWS as the pumping well (MW5 has since been abandoned) and monitoring wells MW1, MW2, and MW7 as observation wells. Transmissivity values for the surficial aquifer of 57 ft2/day and 504 ft2/day were calculated from drawdown data obtained from wells MW1 and MW2, respectively. This corresponds to a hydraulic conductivity for the surficial aquifer at the site ranging from 5.7 ft/day to 36 ft/day. Please let me know if you need any additional information for the approval of the UIC permit for the installation and operation of the proposed iSOC remediation system at the Midway Motors site. Looking at the proposed injection wells (RW-1,3,4, & 5), it appears from the included Table 1 that the wells are screened from 5 to 30 feet (3 to 28 feet for RW-5), a depth interval that would be mostly covered by grout if it was indeed emplaced from 3 to 23 feet. I need this information clarified so that I know in which groundwater zone the dissolved Oxygen will be introduced.
