Dear Evan:
Please find the subject groundwater monitoring report attached. This includes the sampling and reporting
requirements of the above referenced permit, Part V, Section B 1 as well as the reporting requirements of
the overall Tank Farm Remedial Action Plan. Enclosed are: a) Form GW-59 for injection well CIW7 and
monitor well MWTF1CR, b) Form GW-59 for each of the fourteen withdrawal wells, c) piezometer level
monitoring field sheet, and d) the summary operational and maintenance report.
As discussed with you by phone, we have installed a cartridge filter upstream of the UV treatment
equipment. This cartridge filter will replace the multimedia filters, which were supplied with the Culligan
system. Recent testing has revealed that the coliform levels increased across the multimedia filters. With
the installation of the cartridge filter and elimination of the multimedia filters, we expect the UV system to
produce sterile water for injection.

OUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
August 2005 — October 2005
The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Ultraviolet Light Disinfection system
during the period.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The withdrawal water well pumps are monitored daily by the area Maintenance Department and
preventative maintenance work is performed as needed. The total monthly and quarterly withdrawal water
volumes for the period are:

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:
Confined Sand Formation Withdrawal Well (SWW) Pumps
SWW-1: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 10/5,
10/18 and 10/26 flushed out pumps.
SWW-2: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 10/5,
10/18 and 10/26 flushed out pumps.
SWW-3: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 9/27
replaced counter. On 10/5, 10/18 and 10/26 flushed out pumps.
SWW-4: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 10/5,
10/18 and 10/26 flushed out pumps. On 10/25 cleaned pump and lines. On 10/27 replaced pump.
SWW-5: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 10/5,
10/18 and 10/26 flushed out pumps.
SWW-6: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 10/5,
10/18 and 10/26 flushed out pumps. On 10/11 reset pump.

Croatan Formation Withdrawal Well (CWW) Pumps
CWW-1: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 9/12
replaced pump and lines. On 9/15 pulled pump and air blasted well. On 9/21 and 9/22 acid washed well.
On 9/26 air blasted well. On 10/5, 10/18 and 10/26 flushed out pumps.
CWW-2: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 10/5,
10/18 and 10/26 flushed out pumps.
CWW-3: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 10/5,
10/18 and 10/26 flushed out pumps.
CWW-4: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 10/5,
10/18 and 10/26 flushed out pumps.
CWW-5: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 10/5,

CWW-6: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 8/18 reset air valve.
On 9/6 and 9/30 flushed out pumps. On 10/5, 10/18 and 10/26 flushed out pumps. On 10/11 replaced
pump.
CWW-7: On 8/2, 8/11, 8/17 and 8/24 flushed out pumps. On 9/6 and 9/30 flushed out pumps. On 10/5,

On 9/30 change oil, air filter and oil separator on system compressor.
