Part IV: Required Supplemental Information
Submit one (1) hard copy and one (1) electronic copy of a Draft Stormwater Management Plan (SWMP)
with this permit renewal application. The Draft SWMP must be in the current NCDEQ SWMP Template
format and shall include all required information in order for the permit renewal application to be
considered complete.

Part V: Certification
By my signature below I hereby certify, under penalty of law, that this document and all attachments
were prepared under my direction or supervision in accordance with a system designed to assure that
qualified personnel properly gather and evaluate the information submitted. Based on my inquiry of
the person or persons who manage the system, or those persons directly responsible for gathering the
information, the information submitted is, to the best of my knowledge and belief, true, accurate, and
complete.

Dear Ms. Stephenson:
On May 18 —19, 2021, staff from the North Carolina Department of Environmental Quality (DEQ) conducted a compliance
audit of the Fort Bragg - Directorate of Public Works (FB-DPW) National'Pollutant Discharge Elimination System (NPDES)
Municipal Separate Storm Sewer System (MS4) Permit. For your reference, a copy of the MS4 Program Audit Report issued
by DEQ is enclosed with this notice.
The audit identified no significant deficiencies with the specific components of the MS4 permit that were reviewed. The FB-
DPW should self -evaluate the permit components that were not reviewed by DEQ for compliance: Public Education &
Outreach; Public Involvement & Participation; and Construction Site Runoff. Controls.
In accordance with Parts V and VI of the NPDES M34 Permit and DEQ Policy, a new 5-year MS4 permit will be issued in
response to the audit. As such, FB-DPW is required to submit an NPDES MS4 permit renewal application within thirty, (30)
calendar days from the date of receipt of this notice. The application form can be found on the DEQ stormwater web page.
Because the FB-DPW is currently in compliance and the existing Stormwater Management Plan is sufficient, the FB-DPW
is not required to provide the supplemental information in Part IV of the NPDES MS4 Permit Renewal Application Form.
Please attach a copy of this notice to the permit application to fulfill the required supplemental information in Part IV of the
application. The permit renewal application must contain an original "wet" signature and should be submitted by mail to:
