Dear Carmelina Polanco De Ramos:
This letter is to inform you that the subject facility is operating a wastewater treatment system without a valid permit. The Division of
Water Resources records indicate Carmelina Polanco De Ramos currently owns the above subject facility. As the property owner, you
are also the owner of the existing single. family wastewater treatment system, which treats the domestic wastewater from the residence
and releases the effluent to waters of the state.
Part 11 Section B.14 of General Permit NCG550000 requires the permittee to "pay the annual administering and compliance
monitoring fee within thirty days after being billed by the Division." According to Division records, the Annual Fee ($60.00) for
period April 2, 2021 through March 31, 2022 has not been submitted for the subject facility. Payment in the amount of 560 must be
remitted to the Division as indicated on the attached Annual Permit Fee Invoice.
This places Carmelina Polanco De Ramos in violation of North Carolina General Statute § 143-215.1(a)(2), which states that no
person may operate a treatment works or disposal system unless that person has received a permit from the Commission. Failure to
renew the subject permit may result in the assessment of civil penalties of up to $25,000 per violation.
To prevent further action, please submit payment within fifteen (15) days receipt of this letter. If you have documentation proving
that this record is in error, please forward them to our office.
