Liahtsey , air a duly registered Professional Engineer (please .print) in the State of North Carolina and have been authorized to observe (circle one: periodically, weekly, full time) the construction of the subject project for the Permittee: Project No.. AC0005126 Project: Wastewater Improvemennts Project Location: Tyson Foods, Harmony (NC) Rendering Facility Approved Flow: 0.216114.GD (pretreatment system) 0.3 MGD (secondary treatment system) The project consists of the following wastewater treatment improvements at the "skimmer flows" pretreatment system: • construction of one (1) flow equalization basin and one (1) dissolved air flotation (DAF) unit, with chemical storage and feed systems; • construction of a process and laboratory, building, in part to house the DAF unit; and • all associated yard piping, yard structures, lumps, motors, valves, and all other related facilities as described in the plans and specifications. Box 88 Wilkesboro, North Carolina 28697 AVA NCDENR Subject: Authorization to Construct Application No. AC0005126 Permit No. NC0005126 Harmony Rendering Plant Iredell County Dear Mr. Wyatt: The Division of Water Quality received Tyson Foods' request for an Authorization to Construct on March 16, 1998, and additional information on April 13 and May 1. The Division has reviewed the final plans and specifications and the supplemental information for the subject project and found them to be satisfactory. Authorization to Construct. The Division hereby authorizes the following wastewater treatment improvements at the "skimmer flows" pretreatment system: • construction of one (1) flow equalization basin and one (1) dissolved air flotation (DAF) unit, with chemical storage and feed systems; • construction of a process and laboratory building, in part to house the DAF unit; and • all associated yard piping, yard structures, pumps, motors, valves, and all other related facilities as described in the plans and specifications. Upon completion of these improvements and submittal of the Engineer's Certification (see below), the approved treatment capacity of the pretreatment system will be 0.216 MGD. The approved capacity of the secondary treatment (aerated lagoon) system remains at 0.5 MGD. Tyson Foods is authorized to continue its present chemical addition program with the new DAF unit. Before adopting a modified program, Tyson must prepare a design report for Division approval. The report should describe the study method used to evaluate alternate treatments, summarizes the test results, and thoroughly describes the chosen chemical addition program and predicted full-scale performance. Further, the report should identify all materials tested, dosages used, other significant test conditions, and the resulting pretreatment performance. The report shall be submitted to Mr. Mike Templeton of the NPDES Unit at least 30 days prior to full-scale implementation. This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES Permit No. NC0005126 issued July 29, 1994 and modified October 10, 1997, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0005126. Notification. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be P.O. The Permittee shall notify the Mooresville Regional Office, telephone number 704-663-1699, at least forty-eight (48) hours in advance of operation of the installed facilities, so that an in -place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours of 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. Certification. Upon completion of construction and prior to operation of the modified facility, the Permittee shall provide the Division with a certification from a professional engineer certifying that the permitted facility has been installed in accordance with the NPDES Permit, this Authorization to Construct, and the approved plans and specifications. Mail the Certification to the Permits and Engineering Unit, P.O. Box 29535, Raleigh, NC 27626-0535. Continued Requirements. The Operational Agreement between the Permittee and the Environmental Management Commission is incorporated herein by reference and is a condition of this Permit. Noncompliance with the terms of the Operational Agreement shall subject the Permittee to all sanctions provided by G. S. 143-215.6 for violation of or failure to act in accordance with the terms and conditions of this Permit. Failure to abide by the requirements contained in this Authorization to Construct may subject the Permittee to an enforcement action by the Division of Water Quality in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. , The sludge generated from this treatment facility must be disposed of in accordance with G.S. 143- 215.1 and in a manner approved by the North Carolina Division of Water Quality. The issuance of this Authorization to Construct does not excuse the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. A copy of the approved plans and specifications shall be maintained on file by the Permittee for the life of the facility.
