Per the Monitoring and Reporting Requirements (Part VII, page 4) of Injection Permit WI0800264, the well construction
records for the injection wells are to be submitted to the NCDENR Aquifer Protection Section's UIC Program. Injection
wells RW1, RW2, RW3, and RW4 were installed on November 13-14, 2012, and copies of the well construction records
are attached.
Per the Operation and Maintenance Requirements (Part V, page 3) of Injection Permit WI0800264, at least 48-hours
notice must be provided to the NCDENR Aquifer Protection Section's UIC Program prior to the initiation of the injection
operation. We plan to begin installing the inSitu Submerged Oxygen Curtain (iSOC®) system components (i.e., iSOC®
diffusion devices, oxygen cylinders, pressure regulators, filters, tubing, etc.) next Wednesday, December 5, 2012, and
anticipate being able to begin operating the iSOC® system either by the end of Wednesday or on Thursday, December 6,
2012.

operation of the facility. In order to continue uninterrupted legal use of the injection facility for the stated purpose, you
