Good morning Thomas.
Thank you for making the change in BIMS.
am aware of the "Renewal w/Major Mod" option. However, the assignment slip did not specify if application was for a
minor or major modification. It is only marked "Renewal w/Modification". Currently, there is not a "Renewal w/Minor
Mod" option in BIMS. Therefore, I have to know whether this application is a minor or major mod because that will
determine how it is entered into BIMS. If it is a major mod, then I can use the "Renewal w/Major Mod" option. But, if it
is a minor modification, then I have to enter 2 separate applications, 1st -Minor Mod and 2"d-Renewal. The Minor Mod
has to be issued & completed in BIMS 1st before BIMS will allow me to enter the Renewal for this particular application.
Also, I have had other "Renewal w/Modification" packages in the past that were Minor Modifications.
Note: In the future during your review process, on "Renewal w/Modification" application packages, please make sure
to specify whether it is a minor or major modification. This determines how the
application is entered into BIMS.

select the "Renewal with major mod" option and enter the permit number

This is a follow up on our conversation regarding the sampling frequency of the target contaminant (sulfate) for the
subject permit. The amount of injectant fluid (magnesium sulfate) will increase from 800 gallons to 6,000 gallons;
therefore, the amount of sulfate will increase proportionally. The Regional Office does not have concerns keeping the
previous sampling frequency as long as the sulfate concentrations remain below the 2L standard of 250 mg/I.

is proposing to analyze for benzene, MTBE, naphthalene, and sulfate, DO, ORP, pH, temperature, and
specific conductance. The RO recommendation would be to review on current monitoring/sampling frequency
since the dosage of magnesium sulfate @20% has increased from 800 gallons in the 2010 permit application to
6,000 gallons per injection (1,000 gallons/well) in the current application. The target (introduced) contaminant
to be checked is sulfate.

SUBJECT: Use of Non -Biological Products Epsom Salt, Technical Grade (magnesium
sulfate, heptahydrate) and Iron Sulfate to Enhance Biodegradation of
Contaminated Groundwater
I am writing in response to a request for a health risk evaluation regarding the use of non -
biological products including epsom salt, technical grade (i.e., magnesium sulfate, heptahydrate)
and iron. sulfate to enhance biodegradation of contaminated groundwater. It is my understanding
from the information submitted by Delta Environmental Consultants, Inc., that if the dissolved
iron concentration significantly decreases, then the application wil.1 be modified from. magnesium
sulfate to iron sulfate. Based upon my review of the information submitted by Delta
Environmental Consultants, Inc., I offer the following health risk evaluation:
1. Some effects or hazards reported to be associated with the chemicals proposed for use are
as follows:
• Exposure can cause irritation of eyes, nose and throat; early flushing; nausea;
vomiting; hypotension; ECG changes (prolonged PR and QRS intervals); CNS
depression; respiratory depression; impairment of neuromuscular transmission
(hyporeflexia, paralysis); and gastrointestinal hemorrhaging (Micromedex TOMEs
Plus System CD-ROM Database, Volume 70, 2006)
2. If the products are released into the environment in a way that could result in a
suspension of fine solidor liquid particles (e.g., grinding, blending, vigorous shaking or
mixing), then proper personal protective equipment should be used. The application
process should be reviewed by an industrial hygienist to ensure that the most appropriate
personal protective equipment is used.

3. Persons working with this product should at least wear goggles or a face shield, gloves,
and protective clothing. Face and body protection should be used for anticipated splashes
or sprays. Again, consult with an industrial. hygienist to ensure proper protection.
4. Eating, drinking, smoking, handling contact lenses, and applying cosmetics should never
be permitted in the application area during or immediately following application. Safety
controls should be in place to ensure that the -check valve and the pressure delivery
systems are working properly.
5. The Material Safety Data Sheets should be followed to prevent adverse reactions and
injuries.
6. Access to the area of application should be limited to the workers applying the product.
In order to minimize exposure to unprotected individuals, measures should be taken to
prevent access to the area of application.
7. According to the information submitted, the area is served by a public water supply.
There are no public water supply wells within 900 feet down gradient of the site. There
is one on -site non -potable well (used for washout and hydrostatic testing) 160 feet up
gradient from the proposed site of injection. The nearest surface water body is the Neuse
River which is approximately �/4 mile northwest of the site. Efforts should be made to
prevent contamination of existing or future wells and surface waters that may be located
bear the application area.
8. Based on the information submitted, iron sulfide and magnesitun. carbonate are typical
degradation products. An inorganic analysis including iron, sulfate, and total dissolved
so.li.ds should be done before and after the injection of the magnesium sulfate. This is
needed so that a comparison can be made between the naturally -occurring inorganics
prior to injection. and post injection and to ensure that the iron, sulfate, and total dissolved
levels do not exceed the groundwater quality standards of 0.3 mg/L, 250 mg/L, and 500
mg/L, respectively, as a result of the injection.
