Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:* The parcel is currently being developed as a residential subdivision. The crossing in question was originally permitted under USACE ID # SAW 2006-41670-380 4b. Explain the purpose of the proposed project:* The purpose of the proposed project is to repair the existing pipe so that it will safely confer the waters from above the road to below the road and construct taller headwalls to reduce the side slopes adjacent to the roadway for a safer crossing. Additionally, accumulated sediment at the downstream end of the pipe will be removed so the pipe (and stream) can effectively flow downstream. 4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:* The project Will consist of the removal and replacement of approximately 8 linear feet of the existing pipe and construct new headwalls on each end of the pipe to reduce the slopes from the roadway to make the crossing safer. Additionally, the downstream end of the pipe is currently partially blocked With accumulated sediment. This accumulated sediment Will be removed and approximately 20 linear feet of stream will be repaired by removing the accumulated sediment so the stream can flow more efficiently downstream. An additional 20 LF of temporary impact is also proposed to provide ample room for pump around activities during the project. The sediment removed from the stream will be taken to an upland area on site and stabilized. There should be no major downstream indirect impacts from the project. The type of equipment used to perform these tasks include, but are not limited to: Excavators, dump trucks, and similar equipment. 5. Jurisdictional Determinations 5a.
