Dear Mr. McNair:
In accordance with the application received on June 15, 2021, and the additional requested
information received on June 25, 2021, we are forwarding herewith, Permit No. WS0500157 dated June
30, 2021, issued for the construction of a water supply well network (five wells). This Permit will be
effective for one year from the date of its issuance and shall be subject to the conditions and limitations as
specified therein.
Please note this permit does not constitute approval of the volume of water proposed to be
withdrawn within an area defined as existing within a Capacity Use Area, or any permits issued by the local
health department. Additional permits and/or approvals may be necessary and we encourage you to contact
the appropriate agencies prior to constructing any supply wells. If you have any questions, please contact

PERMIT FOR THE CONSTRUCTION OF A WELL
In accordance with the provisions of Article 7, Chapter 87, North Carolina General Statutes, and
other applicable Laws, Rules and Regulations:
PERMISSION IS HEREBY GRANTED TO
Premiere Homes
FOR THE CONSTRUCTION OF A SUPPLY WELL SYSTEM (Bayleaf Master System) consisting
of five water supply wells owned by Premiere Homes at mailing address 8310 Bradford Way, Raleigh, N.C.
The wells will be located on the property owned by Ceres Investments LLC/Carlton Midyette located at
3016 & 3020 Beechtree Camp Drive and 4805 Green Tunnel Lane in Raleigh, North Carolina -Wake County.
This permit is issued in accordance with the application received on June 15, 2021, and the additional
information received on June 25, 2021, in conformity with specifications and supporting data, all of which
are filed with the Department of Environmental Quality, which are considered a part of this permit.
This Permit is for well construction only, and does not waive any provision or requirement of any
other applicable law or regulation. This permit does not constitute approval of the proposed volume of water
to be withdrawn, within an area defined as a Capacity Use Area, overseen by the Division.
Construction of any well under this Permit shall be in strict compliance with the North Carolina
Well Construction Regulations and Standards (15A NCAC 2C .0100), and other State and Local Laws and
regulations pertaining to water supply well construction. Please be advised that some Counties may also
require a permit.
This Permit will be effective for one year from the date of its issuance and shall be subject to other
specified conditions, limitations, or exceptions as follows:
1. Furnish copies of chemical analyses to the Division of Water Resources (DWR), Public Water
Supply Section (PWS), Raleigh Regional Office (RRO), 1628 Mail Service Center, Raleigh, N.C.
27699-1628.
2. Send the results of the aquifer test to the DWR, PWS, RRO 1628 Mail Service Center, Raleigh, NC
27699-1628.
3. Please ensure the contractor responsible for constructing the well is a North Carolina certified water
well contractor.
