4. Project Description and History
4a.Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
SR 1122 Hydeland road is a gravel secondary road that parallels a man made canal.Situated at the head of the canal is a Boating Access Site maintained by the NC WRC.Boat wakes
from traffic utilizing the boat ramp are causing the shoulder to collapse and the erosion is reaching the gravel travel lanes.
4b.Have Corps permits or DWR certifications been obtained for this project(including all prior phases)in the past?*
r Yes r No t: Unknown
4f.List the total estimated acreage of all existing wetlands on the property:
2 acres
4g.List the total estimated linear feet of all existing streams on the property:
5,000
4h.Explain the purpose of the proposed project:*
The purpose of this project is to restore the eroded shoulder to a consistent width and armor the shoulder with a sloping riprap revetment constructed using class B granite. This project
is only addressing areas that have an immediate need for stabilization.
4i.Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
This is a shoulder repair and bank stabilization project.Dump trucks will deliver fill dirt and rock to the site.An excavator will operate from the gravel road to shape the shoulder and
place the riprap.A woven geotextile fabric will be keyed in place once the shoulder has been shaped.Class B granite riprap will then be placed on a 2'to 1'slope to protect the
shoulder.A total of 250 linear feet will be completed addressing only areas that are in need of stabilization to prevent encroachment of the escarpement into the gravel travel lane.
5.Jurisdictional Determinations
5a.Have the wetlands or streams been delineated on the property or proposed impact areas?*
s Yes r No C Unknown
Comments:
USACE Project manager Kyle Barnes delineated the wetlands during a June 25 2021 pre-
application site meeting
5b.If the Corps made a jurisdictional determination, what type of determination was made?*

6. Future Project Plans
6a.Is this a phased project?*
6 Yes O No
6b.If yes,explain.
A Major CAMA/Dredge and Fill permit,RGP 291 and corresponding DWR 401 Certification will be sought to repair and stabile the remaining 2700 linear feet of bank.
Are any other NWP(s),regional general permit(s),or individual permits(s)used,or intended to be used,to authorize any part of the proposed project or related activity?

E. Impact Justification and Mitigation
1.Avoidance and Minimization
1a.Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
A 2':1'slope was chosen to reduce the revetment footprint while maintaining a stable slope.
lb.Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
A geotextile cloth will be utilized to prevent seepage of backfill material as well as prevent settling of the riprap. The riprap will be handled in a manner
to prevent accumulations of sediment or gravel from being placed on the revetment.
2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State
2a.Does the project require Compensatory Mitigation for impacts to Waters of the U.S.or Waters of the State?
✓ Yes ( No

F. Stormwater Management and Diffuse Flow Plan (required by DWR)
1. Diffuse Flow Plan
1a.Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules?
( Yes rNo
What type of SCM are you providing?
r Level Spreader
fJ Vegetated Conveyance(lower SHWT)
r Wetland Swale(higher SHWT)
r Other SCM that removes minimum 30%nitrogen
r Proposed project will not create concentrated stormwater flow through the buffer
2.Stormwater Management Plan
2a.Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250?*
( Yes r No
Comments:
A consistent shoulder width will be constructed and vegetated with grasses to match existing conditions. The project will maintain existing conditions within the buffer and no stormwater
runoff flow will be concentrated and diffuse flow Will be maintained.
