In accordance with a letter dated January 15, 1988, the Division of
Environmental Management is hereby amending Authorization to Construct (No. NC0066966)
to reflect the changes in the dgsign of the previously approved wastewater treatment
facilities. The amended project description and associated conditions are as follows:
"Authorization is hereby granted for the construction of 0.160 :`1GD wastewater
treatment facility consisting of approximately 250 linear feet of 6-inch force main, a
250 GPM wet well pump station equipped with duplex submersible pumps, a high water
alarm, gate valves and a removable barscreen, odophos feed equipment; dual 80,000 GPD
contact stabilization treatment units, each consisting of a 24,970 gallon reaeration
tank and a 19,500 gallon secondary clarifier, a 2,444 gallon aerated sludge holding
tank, a 1,431 gallon chlorine contact tank with gas chlorinator, three 300 cfm blowers
to serve the entire plant; recording effluent flow meter with totalizer; and all
related valves, pipes and appurtenances to serve Quarterstone Farm located in Guilford
County".

The Permittee shall provide for the pump station and force main the following
a. Pump-on/pump-off elevations located so that 2-8 pumping cycles may be
achieved per hour in the pump station.
b. An air relief valve located at all high points along the force main.
c. A screened vent for the wet well.

d. Fillets located in the wet well at the intersection of the flooring
and sidewalls.
e. Three (3) feet of cover (minimum) over the force main or the use of
ferrous material where three (3) feet cannot be maintained.
f. Sufficient devices which will protect the pump station from vandals.

Flood protection if the pump station is located below the 100-year
flood elevation.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer certifying
that the permitted facility has been installed in accordance with the approved plans
and specifications. Mail the certification to the Permits and Engineering Unit, P.O.
Box 27687, Raleigh, N.C. 27611-7687
A copy of the approved plans and specifications shall be maintained on file by
the permittee for five years from the date of issuance
This amendment does not alter any other conditions or limitations specified in
Authorization to Construct (NC0066966) issued June 16, 1988-.
