Dear Mr. Wood:
The Permits and Engineering Unit has completed a preliminary engineering review of the subject
application. The following items must be addressed before we can complete our review:
1) Please show the water line crossing near manhole 14 + 59.54 (sheet 3) on the profile view.
Please submit three revised copies of this sheet.
2) Please be advised that 15A NCAC 2H .0219 requires that either a 10 foot horizontal
separation or an 18 inch vertical separation be maintained from water lines at all points. If
neither of these separations can be maintained, then ductile iron pipe must be specified for
the sewer line. For the following areas, the minimum horizontal separation of 10 feet is not
met, and the vertical separation is not indicated. Please submit three copies of these sheets
to indicate the location of the water line on the profile drawing, and specify ductile iron pipe
as necessary.

b) sheet 4: manhole 29 + 74.44 to manhole 31 + 81.52
3) Please be advised that the sewer lines indicated for Scales Court, Charger Court, Josh
Court, and Tippit Trail will require permitting by the Division. Please either indicate that
sewer extension applications for these streets will be forthcoming, or submit plan, profile,
and a revised application form at this time.
4) The application form indicates that only 28 lots are to be connected to this sewer line.
However, a review of the plans shows that more than 28 lots can be connected directly to
this sewer line. Please revise the wastewater flow rate on page 2 of the application form to
account for all of the lots that will connect directly to this sewer extension.

spacing shall be as follows:
a. 36 feet separation for slopes of 21% to 35%;
b. 24 feet separation for slopes of 36% to 50%;
c. 16 feet separation for slopes of 50% and greater.
For velocities greater than 15 fps, it is strongly recommended that measures be
considered which will protect the sewers and manholes from erosion. For velocities
greater than 20 fps, erosion control measures must be specified.

Required Items
One original and three copies of the completed and appropriately executed application form.
Three sets of detailed plans and specifications signed and sealed by a North Carolina
Professional Engineer. The plans must include a general location map, a plan view of the
sewer extension, a profile of the sewer extension, and trust show the proximity of the
sewer extension to other utilities and natural features. Specifications may be omitted for
delegated authorities. Each sheet of the plans and the first page of the specifications must be
signed and sealed. Three copies of the existing permit if a renewal or modification.
The appropriate permit processing fee, in accordance with 15A NCAC 2H .0205(c)(5).
If the owner/authority of the wastewater treatment facility (\VWTF) that will be accepting
the wastewater flow from this project is different from the applicant of the project, then a
letter must be provided from the owner/authority of the WWTF specifying the volume of
flow that will be accepted. The letter should be a recent letter and should refer to the project
by the same name as that identified on the application and the plans/specifications.
An Operational Agreement (original and two copies) must be submitted if the sewer
extension will be serving single family residences, condominiums, mobile homes, or town
houses and if the subject sewer extension is owned by the individual residents, a
homeowners association, or a developer.

The Division of Environmental Management's Water Quality Section has revised the
Gravity Sewer Non -Discharge Permit Application Form. The form now incorporates the baste
information that the Division feels is necessary in order for the delegated municipalities to perform Li
review on a gravity sewer project for the Division; therefore, the Division will no longer require :hLit
