1. Brief project description: Project begins approx. 3200 feet south

b. Engineer should provide statement of his evaluation of downstream sewers to
accept the wastewater: This system is tributary to the
Beaver Creek Interceptor System which was designed
to include'this Persimmon Creek drainage area.

* The total volume of wastewater to be eventually generated by this project
is estimated to be 1,018,846 gallons per day. This estimate is deter-
mined as follows:
A sewage flow rate of 85 gpcd was derived in the 201 Facilities Plan for
Cumberland County, based on water use of 105 gpcd reduced by 20% consump
tive loss.
The Cumberland County Planning Department estimated 2.9 persons per house-
hold for areas similar in character to the'Persimmon Creek drainage basin.
■ Based on existing development and current trends it is believed that
approximately 2.25 units per acre is a valid projection for the area to
be served by this project.

C. The appropriate permit processing fee, in accordance with 15A NCAC 211.0205(c)(5).
d. If the owner/authority of the wastewater treatment facility (WWTF) that will be accepting
the wastewater flow from this project is different from the applicant of the project, then a
letter must be provided from the owner/authority of the WWTF specifying the volume of
flow that will be accepted. The letter should be a recent letter and should refer to the project
by the same name as that identified on the application and the plans/specifications.
e. An Operational Agreement (original and two copies) must be submitted if .the sewer
extension will be serving single family residences, condominiums, mobile homes, or town
houses and if the subject sewer extension is owned by the individual residents, a
