1. OBTAIN GRADING/EROSION CONTROL PLAN APPROVAL AND PERMIT FROM CITY OF WINSTON-SALEM.
2. CONTACT EROSION CONTROL INSPECTOR AND CITY OF WINSTON SALEM TO ESTABLISH A PRE -CONSTRUCTION
CONFERENCE AND INSPECTION SCHEDULE.
SELF INSPECTION —EFFECTIVE OCTOBER 1, 2010, PERSONS CONDUCTING LAND -DISTURBING ACTIVITIES, LARGER
THAN ONE (1) ACRE, MUST INSPECT THEIR PROJECT AFTER EACH PHASE OF THE PROJECT, AND DOCUMENT THE
INSPECTION IN WRITING ON APPROVED FORMS AVAILABLE FROM THE CITY OF WINSTON-SALEM INSPECTIONS
OFFICE.
1. G.C. TO CLEAR ENOUGH TO INSTALL TEMPORARY SILT FENCE AND OUTLETS AND TEMPORARY CONSTRUCTION
ENTRANCE.
2. G.C. TO BEGIN UTILITY CONSTRUCTION, BUILDING CONSTRUCTION AS SOON AS GRADE IS AVAILABLE. ONCE
VEGETATIVE AREAS REACH GRADE G.C. TO STABILIZE WITH VEGETATION.
3. PRIOR TO THE CONTRACTOR DEMOBILIZING FROM THE SITE (AT APPROXIMATELY 45 DAYS PRIOR TO
DEMOBILIZATION), THE FINANCIALLY RESPONSIBLE PARTY (OR THEIR DESIGNEE) WILL NOTIFY THE
DESIGNATED EROSION CONTROL INSPECTOR OF THEIR ANTICIPATED DATE TO LEAVE THE SITE. AN ON -SITE
INSPECTION WILL BE CONDUCTED PRIOR TO THE LEAVE DATE THE DESIGNATED EROSION CONTROL
INSPECTOR TO MAKE CERTAIN ALL ACTION ITEMS HAVE BEEN ADDRESSED BY THE CONTRACTOR.
4. CONTINUE TO MAINTAIN APPLICABLE EROSION CONTROL MEASURE UNTIL VEGETATIVE COVER HAS BEEN
ESTABLISHED OVER DISTURBED AREAS AND SITE HAS BEEN STABILIZED.
5. INSPECT AND REPAIR ALL EROSION CONTROL DEVICES ON A WEEKLY BASIS AND INSPECT AND REPAIR
IMMEDIATELY FOLLOWING ANY RAINFALL. REPAIR/REPLACE ALL DETERIORATED MATERIALS.
6. G.C. SHALL CONTACT EROSION CONTROL INSPECTOR PRIOR TO ANY REMOVAL OF EROSION CONTROL
DEVICES.
7. PROVIDE TEMPORARY AND PERMANENT GROUND COVER ON EXPOSED SLOPES FOLLOWING COMPLETION OF
ANY PHASE OF GRADING PER NPDES GENERAL PERMIT FOR CONSTRUCTION ACTIVITIES (NCG01000)
8. THE PROPERTY OWNER IS RESPONSIBLE OF MAINTENANCE OF PERMANENT EROSION AND SEDIMENT
CONTROL MEASURES AND GROUND COVER AFTER SITE DEVELOPMENT.
9. G.C. IS RESPONSIBLE FOR STABILIZATION OF ALL EXPOSED GRADES WITHIN 14 CALENDAR DAYS MAXIMUM.

10. CONSTRUCTION SITE POLLUTANTS MUST BE MAINTAINED ON -SITE AND NOT ALLOWED TO REACH THE STATES WATERS. ANY SPENT
FLUIDS FROM CONSTRUCTION TRAFFIC MUST BE CLEANED UP AND DISPOSED OF PROPERLY. ANY HERBICIDE, PESTICIDE AND
FERTILIZER MUST BE USED IN ACCORDANCE TO THE LABEL RESTRICTIONS. ALL WASTES COMPOSED OF BUILDING MATERIALS SHALL
BE DISPOSED OF IN ACCORDANCE WITH NORTH CAROLINA GENERAL STATUES, CHAPTER 130A, ARTICLE 9 - SOLID WASTE
MANAGEMENT, AND RULES GOVERNING THE DISPOSAL OF SOLID WASTE (NC ADMINISTRATIVE CODE SECTION 15A NCAC 13B). NO
