We have been required by USACE to line the stream with Rip Rap down stream of the previously
permitted culverts. Therefore, we need to modify the permit dated October 29, 2012 to include 40 LF of
additional stream impacts.
I am sending you the completed PCN Form, map, and attachments for your review and approval. Please
find enclosed Four (4) packages of the following information along with check in the amount of $240.00:

The purpose of the proposed project is to construct a road and utilities over an existing creek to access Wantiands Ferry
Section I. Due to the configuration of the downstream creek and the outlet velocity from the culverts, Rip Rap will be used
to line the creek bottom and banks for 40 feet.

Restoration Area Notes
1. Remove all deposited fill material within restoration area. Fill in any scour holes or
other holes with topsoil or organic soils, to get grade back to original existing grade.
2. Plant wetland trees and shrubs on 6' centers as shown.
3. Seed area with annual seed (German Millet or Annual Rye) and cover with straw to
diminish erosion while vegetation is establishing.
4. In periods of extreme heat or drought, water area once or twice a week to keep
plants from dying.
5. The restoration area must be monitored once a year for 5 years or until determined
successful by USAGE. Vegetative success rate shall be 65 % survival or 65% aerial
cover of appropriate wetland vegetation. Yearly monitoring is to include a narrative
report with photos submitted to USACE. The first monitoring report will be
submitted 30 days after the planting is completed. If after 3 years, the success
rate is not obtained, a contingency plan will be provided to USAGE.

Install 20 plants, 2 of each tree and number of each shrub shown. Randomly mix and
stagger planting locations on 6' centers as shown. Space trees further apart from each
other and from existing trees, then fill in with shrubs. All trees and shrubs shall be 1
gallon (minimum) in size.

1. For the permitted temporary impact areas, upstream and downstream of the pipes, the
stream, and wetland areas shall be returned to the original contours. The stream banks
shall be seeded then lined with High Velocity Excelsior Matting then staked with live
willow stakes at 3' on center to stabilize the banks and top of bank to 5' beyond top
of bank. All disturbed wetland areas shall be seeded with annual gross and mulched.
