Sorry about that I printed it straight so I'm not sure how the printing was messed up. I assume you are referring to the
pcn forms pages. The copy I have here is not missing anything. I do apologize for the error. The PCN form in its entirety

DWQ received the re- packaged permit application sets today for the above referenced project; however, it
was not copied double sided and DWQ is missing pages 2, 4, 6, 8, and 10 for each set. Rather than further
delay the process, if you are able to scan in those missing pages and email them to me, I will take care of the
rest and move the project forward.
