hart hickman SMARTER ENVIRONMENTAL SOLUTIONS Via Online Portal & 2nd Day FedEx June 23, 2021 North Carolina Department of Environmental Quality Division of Energy, Mineral and Land Resources 450 West Hanes Mill Road, Suite 300 Winston-Salem, North Carolina 27105 Re: DMR September 2021 — NCG030694 — LLFIex, LLC Oracle Flexible Packaging, Inc. (Former) 1300 Cunningham Avenue Winston-Salem, Forsyth County H&H Proiect No. L L F-007 Dear Madam/Sir: On behalf of LLFIex, LLC (LLFIex), Hart & Hickman, PC (H&H) is submitting the enclosed semi- annual stormwater Discharge Monitoring Report (DMR) for the site referenced above in accordance with the industrial stormwater permit (NCG030694). Please note there are benchmark value metal exceedances in Stormwater Discharge Outfall (SDO) samples SDO-003, SDO-004, and SDO-005. Copper was detected found above the benchmark of 0.010 mg/L for SDO-003 (0.0245 mg/L), SDO-004 (0.0150 mg/L) and SDO-005 (0.0162 mg/L). Zinc was detected above the benchmark of 0.126 mg/L for SDO-003 (0.131 mg/L). Historically, the Former Cunningham Plant outfalls have exceeded the benchmarks of zinc and copper. In 2014, DEQ performed an inspection to assess compliance with the NPDES general stormwater permit to evaluate the drainage of the facility's five SDOs. The SDOs were found to be in excellent condition and concluded in a letter dated June 5, 2014 that offsite sources have opportunities to contaminate samples taken from outfalls 1, 2, 4 and 5. Considering the 2014 NC DEQ letter, the only metal exceedance requiring a tiered response is zinc in SDO-003. Because prior results detected no exceedances in SDO-003 above the benchmark for zinc, a Tier One response will be documented in the Stormwater Pollution Prevention Plan (SPPP) in accordance with the industrial stormwater permit (NCG030694). This response will identify potential causes and feasible actions to prevent exceedances in the future. The response will also list monitoring and inspection details such as the date and value of the benchmark exceedance, the inspection date, selected actions, and the date the selected actions are implemented. Additionally, a stormwater management inspection will be conducted to evaluate whether conditions can be improved to prevent exceedances in the future. 2923 South Tryon Street, Suite 100 3921 Sunset Ridge Rd, Suite 301 Charlotte, NC 28203 Raleigh, NC 27607 www.harthickman.com 704.586.0007 main 919.847.4241 main June 23, 2021 Page 2 Should you have any questions or need any additional information, please feel free to contact us. ND - Not Detected at or above adjusted reporting limit. TNTC - Too Numerous To Count J - Estimated concentration above the adjusted method detection limit and below the adjusted reporting limit. MDL-Adjusted Method Detection Limit. Pace Analytical Services, LLC 9800 Kincey Ave. Suite 100 Huntersvifle, NC 28078 (704)875-9092 PQL - Practical Quantitation Limit. RL- Reporting Limit - The lowest concentration value that meets project requirements for quantitative data with known precision and bias for a specific analyte in a specific matrix. S - Surrogate 1,2-Diphenylhydrazine decomposes to and cannot be separated from Azobenzene using Method 8270. The result for each analyte is a combined concentration. Consistent with EPA guidelines, unrounded data are displayed and have been used to calculate % recovery and RPD values. LCS(D) - Laboratory Control Sample (Duplicate) MS(D) - Matrix Spike (Duplicate) DUP - Sample Duplicate RPD - Relative Percent Difference NC - Not Calculable. SG - Silica Gel - Clean -Up U - Indicates the compound was analyzed for, but not detected. Acid preservation may not be appropriate for 2 Chloroethylvinyl ether. A separate vial preserved to a pH of 4-5 is recommended in SW846 Chapter 4 for the analysis of Acrolein and Acrylonitrile by EPA Method 8260. N-Nitrosodiphenylamine decomposes and cannot be separated from Diphenylamine using Method 8270. The result reported for each analyte is a combined concentration. Pace Analytical is TNI accredited. Contact your Pace PM for the current list of accredited analytes. TN -The NELAC Institute.
