On January 8, 2007, the Wetlands/401 Unit of the Division of Water Quality received a Fax/email request
from John Schrum, P.E. of Horvath Associates, PA, regarding a project known as "Glenn View Station
stormwater management plan" for acceptance into the Express Review Program. This letter advises you
that your project will be accepted into the Express Review Program once the following items are
received:
a. The application fee of $1000.00 made payable to the North Carolina Division of Water
Quality;
b. Three (3) complete and collated copies of the Express Review Stormwater Management Plan

c. Three (3) copies of all supporting information pertaining to this project (site development
plans, stormwater management plans, worksheets, etc.);
d. One (1) original and two (2) copies of an Operation & Maintenance Agreement for the
stormwater management facilities.
