Disturbed Acreage:1 Project Information 8 8 8 8 8 0.30 The Entire Project Remarks and Recommendations: Grading Scale:6-0 = Immediate Corrective Action Needed The "ICA" status on this project has been lifted at this time. A follow-up review was conducted on May 13, 2013 with Bill Snell, Shane McGinnis, and Justin Harvel w/DOT and Tim Latham w/Land Quality that indicated corrective actions have been taken to bring the project back into compliance. Please note the following comments and recommendations. 1) Approx. sta.14+50 to sta.18+00 Lt.- contractor has repaired slope drain and has extended special sediment control fence at toe of slope. Temp. seeded/mulched slope is scheduled to be final graded and permanent seeded/mulched/matted next week. 2) Approx. sta.14+50 to sta.20+00 Rt.- erosion in ditch line has been repaired and seeded/matted and erosion control measures have been re-installed, as per plans. 3) Approx. sta.22+50 at end bent 1 - undermining at slope drain has been repaired. 4) *Approx. sta.25+00 Lt.- off-site sediment at corner of wing wall has been removed. As hauling and grading continues in this area, ensure berms, temp. diversions, and slope drains are installed to direct runoff to skimmer basin(ID.- 5.1). Skimmer device appears to be functioning properly at this time. As work progresses, stage seed/mulch/mat slope. 5) *-LOOPC- stream bank has been stabilized with rock, as per DWQ. Special sediment control fence has been installed along this area and disturbed areas are scheduled to be seeded/mulched. 6) Approx. sta.29+50 to sta.32+00 Rt.- special sediment control fence and silt fence have been maintained and runoff has been diverted away from stream area. General comments: The contractor has maintained erosion control devices throughout project. As hauling and grading continues, ensure these activities are coordinated in a manner that runoff will be directed into the skimmer basin by utilizing berms, temp. diversions, and slope drains. Please be reminded that additional e. c. measures may be needed at the end of each workday depending on conditions to prevent off-site in the future. Comments: 10 = Excellent 9 = V.
