Description of project location: The project area is identified as an approximate 25-acre tract, located on Forsyth County,
North Carolina parcels 5897610942 and 5897616961. These parcels are located adjacent to Colerain Creek Drive, in Winston-
Salem, Forsyth County, North Carolina. Work in waters of the United States (U.S.) will occur in Oil Mil Branch, an indirect
tributary to the Yadkin River (8-di2it HUC: 03040101).
Description of projects area and activity: On September 26, 2018, this office provided a Nationwide Permit (NWP) 29
verification to the applicant, Thyme Properties, LLC, who requested a Department of the Army authorization to discharge fill
material into waters of the U.S. in conjunction with the Sa2e Creek subdivision. Project activity involved the construction of
single-family residential lots, alon2 with associated infrastructure and attendant features. Implementation of the project
authorized permanent impacts to 126 linear feet (0.008 acre) of stream channel.
On May 10, 2021, Thyme Properties, LLC, requested a Department of the Army authorization to discharge fill material into
waters of the U.S. in conjunction with the construction of the Crescent Hill residential subdivision. Project activity will involve
the construction of 49 single-family residential lots, alon2 with associated infrastructure and attendant features.
Implementation of the proposed project will permanently impact 69 linear feet (0.01 acre) of stream channel, temporarily
impact 55 linear feet (0.006 acre) of stream channel, and permanently impact 20 linear feet (0.003 acre) of stream channel for
the placement of a riprap dissipater pad (not considered a loss of waters). All other impacts associated with the proposed
project will be conducted in uplands, outside of waters of the U.S. Upon project completion, all temporarily disturbed waters
will be restored to their pre -disturbance conditions.
Cumulative permanent impacts associated with the above referenced subdivisions are 195 linear feet (0.018 acre).
Compensatory mitigation is not required in conjunction with the aforementioned activities.

Your work is authorized by the above referenced permit provided it is accomplished in strict accordance with the attached
conditions and your submitted application and attached information dated 5/10/2021 and design drawings dated 1/21/2021.
Any violation of the attached conditions or deviation from your submitted plans may subject the permittee to a stop work
order, a restoration order, a Class I administrative penalty, and/or appropriate legal action.
This verification will remain valid until the expiration date identified below unless the nationwide authorization is modified,
suspended or revoked. If, prior to the expiration date identified below, the nationwide permit authorization is reissued and/or
modified, this verification will remain valid until the expiration date identified below, provided it complies with all requirements of
the modified nationwide permit. If the nationwide permit authorization expires or is suspended, revoked, or is modified, such that the
activity would no longer comply with the terms and conditions of the nationwide permit, activities which have commenced (i.e., are
under construction) or are under contract to commence in reliance upon the nationwide permit, will remain authorized provided the
activity is completed within twelve months of the date of the nationwide permit's expiration, modification or revocation, unless
discretionary authority has been exercised on a case -by -case basis to modify, suspend or revoke the authorization.
Activities subject to Section 404 (as indicated above) may also require an individual Section 401 Water Quality Certification. You
should contact the NC Division of Water Quality (telephone 919-807-6300) to determine Section 401 requirements.
For activities occurring within the twenty coastal counties subject to regulation under the Coastal Area Management Act (CAMA),
prior to beginning work you must contact the N.C. Division of Coastal Management in Morehead City, NC, at (252) 808-2808.
This Department of the Army verification does not relieve the permittee of the responsibility to obtain any other required Federal,
State or local approvals/permits.
