Dear Mr. Kichefski and Mr. Johnson:
On behalf of our client, the Town of Harrisburg, Kimley -Horn and Associates,
Inc. (KHA) is submitting this request for permit modification from your agencies
in regards to the above referenced project and permit information authorizing the
replacement/upgrade of two undersized piped stream crossings that are in poor
physical condition. The following information is included as part of this
application submittal:
• Revised Project Summary Sheet
• Revised Section 3 of the Pre - Construction Notification Form
• Revised Project Site Figures
• Figure 4a — Carving Tree Lane JD Features and Improvements
• Figure 4b — Leatherwood Lane JD Features and Improvements
• Revised Drawings for the TOWN OF HARRISBURG Culvert
Improvements
In addition to the 75 linear feet of previously authorized permanent impacts, an
additional 26 linear feet of permanent impacts are necessary to place rip -rap at
pipe inlets to stabilize the channel bed and banks and prevent degradation and
instability of the pipe headwalls. The additional rip -rap will be placed to the
minimum extent necessary to reduce discharge velocity and meet non - erosive
design criteria.

Previously authorized temporary impacts will be reduced by 26 linear feet with
the addition of the new permanent impacts. The modified design will result in
permanent impacts to 101 linear feet of a perennial RPW and 101 linear feet of
temporary impacts to install impervious dikes, a pump - around to allow work "in
the dry", and temporary shoulder berms for maintaining neighborhood
access /traffic control.

1. General:
Live stakes are to be mstalled on exposed stream banks as shown on sheets C -03 and C-04. The
Engineer reserves the right to reject any "stakes" that do not meet the above mentioned criteria.
The Contractor shall replace rejected work at no additional cost to the owner
2. Materials:
*Live stakes are to be dormant (cut fall or winter) and gathered locally or purchased from
commercial supplier.
-Stakes must be freshly cut with side branches removed, but with bark intact (ensure that the bark
is not shipped during the cutting, preparation, or installation of the stake).
•Cuttings arc to be collected using a saw or pruning shears (not an ax)
3 Preparabon-
•One end must be cut at an angle for insertion into the soil and the other end must be cut square
for tampmg
*Cuttings must be kept fresh and moist prior to installation.
•Cuttings must be prepared from 0 5 to 2 -inch diameter stock and approximately 2 to 3 feet in
length.
4 Installation:
• Instal l stakes the same day they are prepared
• Installation should occur only between November 15 and May 15.
•Start the installation nearest to the stream and work up the bank/floodplain.
•Cuttings must be installed right side up with the buds pointing upward.
•Cuttings should be tamped into the ground at right angles to the slope and angled slightly
downstream
•They are to be tamped into the ground for approximately 4/5 of their length
*Cuttings that split or become "mushroomed" must be replaced.
*Stakes should be installed in a random configuration to prevent gullies and promote a more
natural effect in the re- vegetated area
