(KHA) is submitting this request for permit modification from your agencies in regards to the above referenced project and permit information authorizing the replacement/upgrade of two undersized piped stream crossings that are in poor physical condition. The following information is included as part of this application submittal: • Revised Project Summary Sheet • Revised Section 3 of the Pre - Construction Notification Form • Revised Project Site Figures • Figure 4a — Carving Tree Lane JD Features and Improvements • Figure 4b — Leatherwood Lane JD Features and Improvements • Revised Drawings for the TOWN OF HARRISBURG Culvert Improvements In addition to the 75 linear feet of previously authorized permanent impacts, an additional 26 linear feet of permanent impacts are necessary to place rip -rap at pipe inlets to stabilize the channel bed and banks and prevent degradation and instability of the pipe headwalls. Previously authorized temporary impacts will be reduced by 26 linear feet with the addition of the new permanent impacts. The modified design will result in permanent impacts to 101 linear feet of a perennial RPW and 101 linear feet of temporary impacts to install impervious dikes, a pump - around to allow work "in the dry", and temporary shoulder berms for maintaining neighborhood access /traffic control.
