Everything looks good except for a few small issues: Please correct the following: - Prior Comment 2 (Original Comment 16a) – Please provide an electronic copy of the O&M Agreement Form (Signed 3/30/21) and the Application, both from the second (April 2021) submission. The electronic copies of these items were not provided in the latest resubmission and are required prior to the project being approved. If you do not have an electronic version of these files to upload, please let me know. - Supplement-EZ Form: o Drainage Areas Page, Entire Site Column:  Line 15 – Please include this value. This value represents the entire amount of BUA that will remain on-site from existing conditions to proposed conditions.  Line 16 – Correct me if I’m wrong, but I don’t believe that any of the existing BUA associated with this site is covered under a different/prior permit. If there is no prior permit then this value would be zero or N/a.  General – The sum of the New BUA (Items in Line 12) and the Existing BUA that will Remain (Line 15) should equal the Total BUA in the Project (Line 8). o Wet Pond Page:  Line 28 – The average depth of the main pool appears to be incorrectly calculated in the SIA (and therefore incorrectly shown in the Supplement-EZ Form). The “Abottom shelf” value in the calculations should be the cross-sectional area of the main pool at the bottom elevation of the vegetated shelf. The bottom of the vegetated shelf is shown at elevation 250.5 in the plans and calculations and the cross-sectional area of the main pool at this elevation is 3,298 sf. This should result in an average depth closer to 4’ (This change shouldn’t necessitate the wet pond design being changed, it appears to just be a calculation/reporting change). Please provide the following: - 1x hard copy & 1x electronic copy of the revised Supplement-EZ Form - 1x hard copy & 1x electronic copy of the revised calculations th - 1x electronic copy of the O&M Agreement provided on April 5 th - 1x electronic copy of the Application provided on April 5 Hard copies should be mailed to me at the following address: (FedEx/UPS) Suzanne McCoy 512 N.
