Pursuant to 15A NCAC 02T .0106, please have an executive officer or elected official complete the attached signature delegation letter before Mr. Chris Phelps can sign the application. Otherwise, please update application form Section I and have the executive office or elected official sign the application form. The application rates listed under Section VIL8. are not consistent with the May 4, 2020 issued permit. Please clarify and update the application form as needed. B. Property Ownership Documentation: 1. No comments. C. Engineering Plans: 1. No comments. D. Specifications: 1. No comments. E. Engineering Calculations: 1. The application rates used in the calculations are not consistent with the May 4, 2020 issued permit. Please clarify and update the calculations as needed. 2. Please clarify whether the 662,216 gallons per day (GPD) is the design facility influent flow or the long-term average influent flow. F. Site Map: On Sheet 1 of Attachment H, there are portions of Field 113, 613, 7A, and 7B within the spray buffer. Please clarify. 2. According to the permit issued May 4, 2020, the existing monitoring well MW-3 is located north of Field 613, while it showed up south of Field 3B on the site map. According to the same permit, the existing MW-9 is located west of Field 7A, but it is not found on the site map. Please clarify. PMW-1 and PMW-2 are close to MW-3 and PMW-3 is close to MW-9. Please clarify whether PMW-1 through PMW-4 are proposed compliance monitoring wells or currently active wells as indicated in the application form Section IV.10. to be added to the permit in addition to the existing compliance monitoring wells MW-3, MW-8, and MW-9. If this is the case, please rename PMW- 1 through PMW-4 as MW-10 through MW-13 and reflect the names on the application form and the site maps.
