Pursuant to G.S. 143-215.6A.(f), the Division of Water Resources has reviewed your remission request and
supporting documentation received April 1, 2021, and have not found justification to modify the initial civil
penalty assessment of $296.35.
Within 30 days of receiving this notice, you shall do one of the following two options:
1. Submit civil penalty assessment payment:
Payment shall be made in the form of a check, money order, or electronic funds transfer made payable
to the North Carolina Department of Environmental Quality in the amount of $296.35. Payment
resolves this civil penalty; however, it does not prevent further enforcement actions for continuing or
new violations. Please submit payment to the following address:

If payment is not received within 30 days of receiving this letter, your civil penalty remission request

Remissions (Committee) for final agency decision pursuant to G.S. 143-215.6A.(f).

meets one or more of the statutory factors, please complete and return the attached form within 30 days
of receiving this letter. All documentation shall be submitted to:

The EMC Chairman will review your oral presentation request and supporting documentation to
