' Monthly sampling (instead of semi-annual) must begin with the second consecutive benchmark exceedance for the same parameter at the same outfall.
For sampling periods with no discharge at any single outfall, you must still submit this discharge monitoring report with a checkmark here.
'The total precipitation must be recorded using data from an on -site rain gauge. Unattended sites maybe eligible for a waiver of the rain gauge requirement.
See General Permit, Section 8, Table 1 to identify the especially sensitive receiving water classifications where the more protective benchmark applies.
5 Total Toxic Organics sampling is applicable only for those facilities which perform metal finishing operations, manufacture semiconductors, manufacture
electronic crystals, or manufacture cathode ray tubes. For purposes of this permit the definition of Total Toxic Organics is that cleflnition contained in the EPA
Effluent Guidellnes for the facility subject to the requirement to sample (for metal finishing use the definition as found In 40 CFR 433.11; for semiconductor
manufacture use the definition as found in 40 CFR 469.12; for electronic crystal manufacture use the definition as found in 40 CFR 469.22; and for cathode ray
tube manufacture use the definition found in 40 CFR 469.31).

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TTO monitoring
may be waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed In the General
Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics
(1TO), I certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the
solvent management plan included in the Stormwater Pollution Prevention Plan."

Please accept the late filing of the Stormwater Discharge Monitoring Report. I was asked to be a
part of this ongoing process. We were all set for our first sample period for 2020. We received
our sampling kit and waited for a measurable storm event which allowed us to take samples on
June 10, 2020. 1 then hand carried them to Face Analytical in Asheville the next day.
Here is where my problems began. I had earlier finished treatment for cancer. Before I received
the test results from Pace Analytical, my wife had to have major surgery in Charlotte. Then four
weeks later she had a second major surgery also in Charlotte. All this involved numerous follow
up trips. As if this were not enough I battled kidney stones while helping take care of my Dad
with dementia while he was hospitalized and died.
Then, my sister and I were working on our Dad's estate when she died unexpectedly the week
before Thanksgiving which left me as the primary care giver for my Mom. I have struggled
trying to work enough to keep my job.
This pandemic has been hard enough on our business. Needless to say, my personal situation
added to that has made for a nearly impossible situation. I'm not trying to just give you a hard
luck story but I need your understanding and help to move forward.
I ordered and received the testing kit in mid -December for the second half of 2020. We have not
had a measurable storm event since that time. We have had more snow than rain. The rain has
not come within working hours.
