Division of Water Resources INSTRUCTIONS FOR FORM CSA 04-16&SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General—When submitting an application to the Pretreatment,Emergency Response,&Collection Systems(PERCS)Unit,
please use the following instructions as a checklist in order to ensure all required items are submitted. Adherence to these
instructions and checking the provided boxes will help produce a quicker review time and reduce the amount of requested
additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.
A. Cover Letter
® Submit a cover letter listing all items and attachments included in the permit application package
B. No Application Fee Required
> No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit
> The appropriate annual fee for systemwide wastewater collection system permits may be found at:

C. System-Wide Wastewater Collection System (FORM: CSA 04-16)Application:
® Submit the completed and appropriately executed System-wide Wastewater Collection System (FORM: CSA 04-
16) application. Any unauthorized content changes to this form shall result in the application package being
returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.
❑ If the Applicant Type in Section I.3 is a Privately-Owned Public Utility, provide the Certificate of Public
Convenience and Necessity(CPCN)from the North Carolina Utilities Commission demonstrating the Applicant is
authorized to hold the utility franchise for the area to be served by the wastewater collection system, or
❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an
application for a franchise has been received and that the service area is contiguous to an existing franchised area
or that franchise approval is expected.
❑ If the Applicant Type in Section 1.3 is a corporation or company, provide documentation if it is registered for
business with the North Carolina Secretary of State.
D. General Information:
> The Authorized signing official listed in Section I.4 should match with that of the Applicant certification page in
accordance with 15A NCAC 02T .0106(b). Per 15A NCAC 02T .0106(c), an alternate person may be designated
as the signing official if a delegation letter is provided from a person who meets the criteria in 15A NCAC 02T
.0106(b).
> NOTE - Public Works Director's are not authorized to sign this permit application according to the rule
unless they are delegated.

SSO EMERGENCY RESPONSE PLAN
1.0 PURPOSE
The purpose of this procedure is to establish the necessary steps to ensure the prompt
and adequate response by Brunswick Regional Water & Sewer H2G0 to overflows and
spills from the sanitary sewer collection system to minimize potential negative health
and environmental impacts and to comply to with applicable State of North Carolina and
USEPA regulations.
2.0 ORGANIZATIONS AFFECTED

3.0 KEY DEFINITIONS
3.1 Sanitary Sewer Overflow (SSO) —ANY leak or other discharge, regardless of
cause, of sanitary sewer liquid and/or solids from any Brunswick Regional
Water & Sewer H2G0 sanitary sewer main, manhole, street clean-out, pump
station or other component of the Brunswick Regional Water & Sewer H2G0
sanitary sewer system or leak of public sewage through any private service.
3.2 Waters of the State of North Carolina —Any stream, creek, river, wet ditch,
pond, lake, swamp, or any other body of water, permanent or temporary, in
place at time of the SSO to that body of water.
3.3 Supervisor—The "ranking" Maintenance Department person on-site
representative responsible at a particular SSO response, whose actual job
classification may vary.
4.0 RESPONSIBILITIES
4.1 The sewer collection system Operator in Responsible Charge (ORC), the
back-up ORC and H2GO's Executive Director are primarily responsible for
SSO's from the sewer collection system.
5.0 PROCEDURE
5.1 Initial Notification and Relay
5.1.1 Normal Working Hours
5.1.1.1 During normal working hours, calls regarding possible SSO's
will be received by H2GO's Office at 910-371-9949 or the
Collection System ORC at 910-279-3409. Calls received by,
and possible SSO problems found by, District employees at
other locations will be immediately relayed to one of those
numbers. The recipient of the call will obtain as much
information as possible about the problem including the
exact location, duration of the SSO (if known), caller name
and telephone number, and any other information that may
assist in the final resolution of the problem, such as other
persons observed in the area of the SSO.
5.1.1.2 If the SSO is clearly a collection system problem or is
unknown, the personnel in maintenance receiving the call
will immediately dispatch an available sewer jet or vac truck
to the call location. The initial personnel and equipment
dispatched may be adjusted based upon the information
received from the initial caller.
5.1.1.3 If the SSO is clearly attributable to a problem at a sewer
pump or lift station, the call will be relayed to the ORC or
back-up ORC.
5.1.2 After Normal Working Hours
5.1.2.1 After normal District working hours, calls regarding possible
SSO's will be received by the On Call person at telephone
number 910-367-2084, or any other number by which the
caller is able to reach. Possible SSO's of which personnel of
other District departments become aware after hours will be
immediately relayed to the Collection System ORC.
5.1.2.2 All of the response personnel may be reached by mobile
phone. Copies of the duty roster are distributed to all
personnel.
5.2 Initial Response
At all times, the appropriate response personnel, as indicated in 5.1.1 and
5.1.2, will respond to the location of the reported possible SSO within a
maximum of Thirty minutes from the initial receipt of the call. The report on
the SSO, as elaborated in section 5.8.1, must include an indication of the time
of initial call receipt and the time that the first representative arrives on the
scene. In the event that the response time is over 30 minutes, an explanation
of the response time must be given. In no case may the response time be over
one (1) hour.
5.3 Initial Assessment
5.3.1 The initial collection system respondent will make an immediate
assessment of the SSO condition. In the event that the incorrect
personnel have been dispatched, the respondent will directly or
through the appropriate communications channels, as set forth
above, notify the appropriate personnel. If it is at any point
determined that the SSO is wholly private in nature, the personnel
will begin efforts to notify the property owner or manager. Each
collection system personnel must be familiar with this procedure
and capable of full compliance with this procedure without
supervisory assistance, except as provided for in this procedure.
5.3.2 Private SSO problems will be referred by the sewer ORC or
maintenance supervisor on the next working day to the Brunswick
County Health Department Environmental Health Section. Those
private SSO's that appear to meet the DENR reporting level will
also be reported to DENR at telephone number (800) 858-0368 or
(919) 733-3300 on the next working day. Problems with the lack
of responsiveness of residential rental property owners will also
be referred to the local building authority.
5.3.3 Any SSO of sewage from H2GO's collection system through a
private service, clean-out or building shall be treated as H2GO's
SSO.
5.3.4 In the event that the SSO isprivate and relativelyserious in nature
and the collection system respondent believes that he/she can
reasonably attempt and likely succeed in making an initial
correction of the SSO, he/she may attempt to do so. In such
cases, collection systempersonnel shall keepaccurate records of
the cost of the work for possible recovery from the responsible
party.
5.3.5 In all collection system SSO's, collection system personnel will
complete Initial Response Evaluations, as included in Appendix 6.6
of this standard procedure, and submit that form to the collection
system ORC.
5.4 Initial Corrective Action
5.4.1 Throughout the response and correction of all SSO's, priority
emphasis must be placed upon public health, employee safety
and environmental protection. Sufficient measures must be taken
to keep the public out of the area contaminated by the SSO.
Depending upon the location and circumstances of the SSO, this
may include barricades and work zone tape or other appropriate
devices. This public restriction should be maintained throughout
the SSO response and for at least 24 hours after the SSO
correction and clean-up is complete. Downstream usage
evaluation and notification are further outlined in section 5.6.3.
All employees must wear all applicable personal protective
equipment (PPE) and must utilize all other applicable safety
equipment, such as gas monitors, confined space entry
equipment and/or trench shoring equipment. Every effort should
be made to contain the sewage material and any disturbed
earthen material and prevent any more from entering any waters.
5.4.2 The initial responding collection system personnel should take
immediate corrective measures, remove the blockage and contain
the SSO if possible. Corrective measures may include attempting
to jet the stopped sewer main or service, resetting a pump,
manually starting a generator, or other measures as may be
appropriate based upon the specific incident. If additional
personnel or equipment, such as a back-up pump, a portable
generator or other specialized equipment, as listed in Appendix
6.2 of this standard procedure, are needed to take the initial
corrective measures, they will be obtained immediately.
5.4.3 In the event that a relatively immediate correction of the problem
is not possible due to a main or manhole collapse or wash-out,
extreme blockage, acute pump station failure or other cause the
collection system personnel will obtain the necessary materials
and equipment to construct a dam to contain the SSO and to
initiate a pumping operation. Depending upon the specific SSO,
the dam may range from a small straw dam to a large earthen
dam. The pumps may range from the 3" portable pumps to the

larger 6" pumps. In some cases, it may be possible to return the
flow from the SSO to the sewer system by gravity until the final
repair can be made. Pumping will continue on a 24-hour per day
basis, with at least one person at the location at all times until the
SSO is corrected.
5.4.4 In some cases, it may be necessary to contact one of the pump-
and-haul or waste oil recovery contractors on retainer with H2G0
to remove and transport the material to an appropriate location
to return it to the sewer system. A list of the current contractors
and the means to contact them will be maintained by the sewer
collection system ORC and provided as updated to all involved in
this procedure. A copy is included in section 6.2 of the Appendix
of this standard procedure.
5.4.5 In the event that the SSO is from a sewer force main outside of a
pump station site, repair responsibilities rest with the ORC or
back-up ORC who will coordinate the shut-down of the pump
station as may be needed to "move" the SSO to a more
acceptable location and/or to facilitate the repair of the main.
5.4.6 In the event that the SSO is due to a rainfall event, the responding
personnel will take any mitigation measures as may be possible,
such as the erection of a straw dam. The personnel will respond to
each reported SSO location and will make a log of all SSO locations
due to the rainfall event including the location and number of
manholes involved, the approximate start and ending times of the
SSO's, the estimated volume, as discussed in section 5.6.7, and any
other relevant information.
5.5 Additional Corrective Measures
5.5.1 In cases in which additional corrective measures are necessary, such
as the repair of a damaged main, collection system respondents will
contact the necessary personnel to perform the repair.
5.5.2 During non-working hours, the necessary equipment operators can
be contacted as listed on the duty roster and the additional
equipment operators list, which is included in Appendix 6.3 of this
standard procedure, as maintained by the Assistant District
Director. In the case of on-going SSO's, corrective measures will
begin immediately and continue 24 hours per day until corrective
measures sufficient to stop the SSO are complete.
5.5.3 In extreme, unusual SSO incidents, such as following a hurricane
incident, contractors under contract with H2G0 may be called in to
perform emergency repairs.
5.6 Additional Notifications
5.6.1 DENR— MUST be notified of any SSO reaching the waters of the
state, regardless of the volume; any SSO over 1000 gallons that
does not reach the waters of the state; or any SSO in which the
volume and/or waters impact of the SSO is unclear.
5.6.1.1 Normal workinghours—Wilmington Regional Office at

information that may be necessary to a fast response.
5.6.1.2 After normal working hours—(800) 858-0368 or (919) 733-
3300
5.6.1.3 The notification to DENR should be accomplished as soon
as practical but, in no case, will the notification to DENR be
more than 24 hours from the initial SSO notification to
H2GO. Timely notification of DENR is very important.
5.6.1.4 In all calls to DENR, the Maintenance Department
representative will log the time of the call, the name of the
state call recipient and any other noteworthy information.
5.6.1.5 In the event that findings or conditions regarding the SSO
location, the ORC will openly review the facts of the
situation with the DENR representative and will attempt to
perform any corrective mitigation measures requested by
the DENR representative.
5.6.1.6 In any SSO incident in which the SSO continues for 24
hours or more beyond the initial report of the SSO to
H2GO, additional reports will be made to DENR each 24
hours until the SSO is corrected.
5.6.2 Maintenance Department Normal Working Hours—ORC
After Normal Working Hours—On-call duty person as listed on duty
roster.
In many cases, the ORC or back-up ORC will be part of the initial SSO
response. The ORC will notify H2GO's Executive Director of all SSO's
that meet the DENR reporting requirements, during normal working
hours.
5.6.3 Downstream Users—As soon as practical, but not more than 24
hours from the initial notification, the ORC will travel downstream
from the SSO to determine whether any downstream uses, such as
fishing, swimming, irrigation or water intake may be impacted by
the SSO and should be restricted. If it is determined that
restrictions may be warranted, collection system personnel should
put physical controls, such as barricades, in place and/or notify the
person or agency responsible for that site.
5.6.4 Executive Director's Office—The ORC or person on call will notify
the Executive Director of all serious SSO's, such as those involving
fish kills, downstream use restrictions, unusual public involvement
or other unusual factors.
5.6.5 Laboratory Personnel —As deemed necessary by the collection
system personnel on site, wastewater plant personnel may be
called to the location to take water samples. Generally, these
samples, when taken, will be taken from upstream and downstream
of the SSO location and will include the analysis of ph, conductivity
and fecal coli form. Additional samples and test may be determined
to be necessary until the downstream water quality returns to that
of the upstream.
5.6.6 Necessary Information —The information provided to the parties
listed above will include any necessary information, generally
including location of the SSO, estimated start and duration of the
SSO, estimated SSO volume, cause if known, correction and clean-
up status.
5.6.7 Volume Estimation
The estimation of the volume of an SSO is based upon the duration
and the rate of the overflow. The preferred method for
determining duration is from eyewitness accounts. The secondary
method is to look at surrounding conditions, such as the impact
upon vegetation. The preferred method for determining the rate of
discharges is to count the number of services on the line, adjust by
the percentage the flow that is overflowing and to multiply by 210
gallons per day per service. This is then multiplied by the duration
to get the total volume of the SSO. Estimating can also be done
based on the total volume contained, if applicable, by looking at the
upstream and downstream flows and the capacity of the pipe, and
by the direct estimation of the rate at which the sewage can be
seen discharging. In the case of pump station SSO's, it may be
possible to estimate the volume based upon the normal pumpage
and the pump station down-time.
5.7 Clean-up
5.7.1 As soon as practical during the SSO response, responding personnel
will gather all fish of larger than two inches in length that may have
been killed during the SSO. This will be performed using nets and
boats if necessary. A detailed log must be maintained of the fish
collected by species, length and quantity. An estimate should be
made of the quantity of dead fish smaller than two inches and not
collected. Whenever a fish kill has occurred, personnel on site
should carefully observe and note any surviving fish or other
aquatic life, as most fish kills involve only a small percentage of the
fish population.
5.7.2 In the first stage of the clean-up, collection system personnel will
remove all visible solids from the ground and the water. This will be
accomplished using hand tools such as shovels and rakes, pumping,
jet flushing and vacuuming, or with heavy equipment as necessary.
5.7.3 All visually contaminated water will be removed by pumping back
into the sanitary sewer system, by vacuuming with a jet/vac truck,
or contract pumping and hauling. As the contaminated water is
removed, fire hydrants will be flowed upstream if possible to
introduce clean water.
5.7.4 The ground area where the SSO occurred will be neutralized by the
application of a heavy layer of powdered limestone. Based upon
the specific SSO situation, sodium bicarbonate (soda) or sodium
hypochlorite (bleach) may also be used. Care must be exercised to
minimize the entry of these products into the water, as they can
have a significant negative effect upon aquatic life.
5.7.5 The flushing of upstream hydrants will be continued until a
complete turnover of the potentially contaminated downstream
waters has occurred.
5.7.6 If necessary, additional stream bank stabilization work or other
general restoration work will be performed at the conclusion of the
clean-up phase.
5.7.7 If necessary, one to seven days following the application of the
limestone or other neutralizing product, the area will be tilled or
plowed and seeded or otherwise returned to its original state, as
possible.
5.8 Follow-up Reporting and Record keeping
5.8.1 As soon as possible, but not more than five calendar days following
the SSO, the sewer ORC will provide to DENR via fax or email a
complete report on any SSO which must be initially reported to
DENR, as indicated in section 5.6.1 of this procedure, on the
reporting form included in Appendix 6.5 of this procedure. They
should also call DENR to confirm that the report has been received.
A copy of the DENR report will also be provided to the Executive
Director.
5.8.2 The sewer ORC will also provide a follow-up report to any other
parties who were notified in the initial SSO notification as indicated
in section 5.6 of this procedure.
5.8.3 If it has been determined that the SSO was caused in whole or in
part by residential grease, the ORC will provide to the Compliance
Director a map of the upstream drainage area. The Compliance
Director will prepare and send a grease education letter to all
property owners in the area.
5.8.4 If it has been determined that the SSO may have been caused in
whole or in part by commercial grease, the ORC will provide to the
Compliance Director a map of the upstream area. The Compliance
Director will identify and contact all restaurants and other potential
grease contributors and conduct additional follow-up as necessary
to ensure that they are in compliance with the grease section of the
Sewer Use Ordinance and the Fats, Oils, and Grease Policy.
5.8.5 If one or more parties have specifically and negligently responsible
for causing the SSO, the Executive Director will prepare and send a
fine letter to those parties.
5.8.6 The ORC will conduct a follow-up evaluation of the causes of any
collection system SSO, including a TV inspection of the main
involved. The ORC will place the location on the appropriate
scheduled maintenance list and will notify the Executive Director if
further engineering evaluation appears warranted.

Subject: Reporting Requirements
Senate Bill 729 ratified by the North Carolina General Assembly on August 20th, 2014 and effective
September 20th, 2014 revised reporting requirements for entities that own or operate wastewater
treatment and collection systems. This letter is provided as guidance to assist you in meeting the
revised reporting requirements. This letter also replaces and supersedes the October 19th, 1999
reporting memo issued by the Division of Water Resources (Division).
G.S. 143-215.1C(a): Report to wastewater system customers on system performance.
No changes were made to this reporting requirement. An annual report for wastewater collection
systems or treatment works is still required for systems that have an average annual flow greater
than 200,000 gallons per day. Additional information related to the annual report can be found on
the Division's website.
G.S. 143-215.1C(al): Report discharge of untreated wastewater to the Department.
This section was added and requires the owner or operator of any wastewater collection or treatment
works to report a discharge of 1,000 gallons or more of untreated wastewater to the surface waters
of the State to the Department as soon as practicable, but no later than 24 hours after the owner or
operator has determined that the discharge has reached the surface waters.
Note that all System-Wide Collection System permits (both deemed and permitted systems), as per
G.S. 143-215.9E are required to report a discharge to the Division of Water Resources no later than
24 hours following first knowledge of the occurrence of either of the following:
• Any SSO and/or spill over 1,000 gallons or greater to the ground surface
• Any SSO and/or spill,regardless of volume that reaches the surface water
The Division will continue to utilize this more stringent reporting requirement in accordance with
15A NCAC 02T .0113(c) & 15A NCAC 02B.0506 and the applicable individual system-wide
collection system permit condition.
Owners or operators of any wastewater collection or treatment works not permitted under the
System-Wide Collection System program must continue to report per any existing permit
requirements.

G.S. 143-215.1C(b)(1): Publication of Notice of Discharge of Untreated Wastewater
This section was modified to require issuance of a press release 24 hours (originally 48 hours) after
the owner or operator has determined that the discharge has reached surface waters of the State.
This applies to a discharge of 1,000 gallons or more of untreated wastewater to the surface waters of
the State. No other modifications were made. For these situations,the owner or operator must:
• Issue a press release to all print and electronic news media that provide general coverage in
the county where discharge occurred
• Set out details of the discharge
• Issue the press release within 24 hours after it has been determined the discharge has
reached the surface waters of the State
• Retain a copy of the press release and a list of the news media it was sent to for at least one
year
• Make the release available to the public upon request
G.S. 143-215.1C(b)(2): Publication of Notice of Discharge of Untreated Wastewater
This section was not modified by Senate Bill 729, however the October 19, 1999 memo issued by
the Division allowed the owner or operator to notify the county in which the discharge occurred and
the county immediately downstream for discharges less than 1,000,000 gallons.
Effective September 20th, 2014, the Division will require owners or operators to adhere to a more
stringent interpretation of the general statute. In the event of a discharge of 15,000 gallons or more
of untreated wastewater to surface waters of the State,the owner or operator must:
• Publish a notice of the discharge in a newspaper having general circulation in the county in
which the discharge occurs and in each county downstream from the point of discharge that
is significantly affected by the discharge
• The Secretary (or delegated authority) shall determine which counties are significantly
affected by the discharge and shall approve the form and content of the notice and the
newspapers in which the notice is published.
• Publish within 10 days after the Secretary (or delegated authority) has determined the
counties that are affected by the discharge and approved the notice and newspapers in which
the notice is to be published.
• File a copy of the notice and proof of publication with the Division within 30 days after the
notice is published. •
• In addition to the Notice,requires a press release per subdivision(1) of this subsection.
G.S. 143-215.1C(c)(1)& (c)(2): Publication of Notice of Discharge of Untreated Wastewater
These sections, applicable to the owner or operator of a non-municipal, non-domestic wastewater
collection or treatment works,were modified in the same mariner as G.S. 143-215.1C (b) (1) and(b)
(2), above and the same notification procedures are required.
Press Release Format
General Statute 143-215.1 C requires that the owner or operator of any wastewater collection or
treatment works to issue a press release when an untreated wastewater discharge of 1,000 gallons or
more reaches surface waters.
In accordance with that regulation, the following news release has been prepared and issued to
media in the affected county:
The (Utility, Municipality, District, etc.) had a discharge of untreated wastewater on (date) of an
estimated (amount) gallons at (location). The untreated wastewater was discharged into (creek,

The Division of Water Resources was notified of the event on July 1, 2014 and is reviewing the

approximately 5 hours. The untreated wastewater entered into Tuner's Creek in the Savannah River
Basin. The pump station was repaired and placed back into serve by replacement of a pump to
prevent further discharge. This notice was required by North Carolina General Statutes Article 21,

PUMP STATION CONTINGENCY PLAN
7.0 PURPOSE
The purpose of this procedure is to establish the necessary steps to ensure the prompt
and adequate response by Brunswick Regional Water & Sewer H2G0 to overflows and
spills from the sanitary sewer collection system to minimize potential negative health
and environmental impacts and to comply to with applicable State of North Carolina and
USEPA regulations.

9.0 KEY DEFINITIONS
9.1 Sanitary Sewer Overflow (SSO) —ANY leak or other discharge, regardless of
cause, of sanitary sewer liquid and/or solids from any Brunswick Regional
Water & Sewer H2G0 sanitary sewer main, manhole, street clean-out, pump
station or other component of the Brunswick Regional Water & Sewer H2G0
sanitary sewer system or leak of public sewage through any private service.
9.2 Waters of the State of North Carolina —Any stream, creek, river, wet ditch,
pond, lake, swamp, or any other body of water, permanent or temporary, in
place at time of the SSO to that body of water.
9.3 Supervisor—The "ranking" Maintenance Department person on-site
representative responsible at a particular SSO response, whose actual job
classification may vary.
10.0 RESPONSIBILITIES
10.1 The sewer collection system Operator in Responsible Charge (ORC), the
back-up ORC and H2GO's Executive Director are primarily responsible for
SSO's from the sewer collection system.
11.0 PROCEDURE
11.1 Initial Notification and Relay
11.1.1 Normal Working Hours
11.1.1.1 During normal working hours, calls regarding possible
SSO's will be received by H2GO's Office at 910-371-9949 or
the Collection System ORC at 910-279-3409. Calls received
by, and possible SSO problems found by, District employees
at other locations will be immediately relayed to one of
those numbers. The recipient of the call will obtain as much
information as possible about the problem including the
exact location, duration of the SSO (if known), caller name
and telephone number, and any other information that may
assist in the final resolution of the problem, such as other
persons observed in the area of the SSO.
11.1.1.2 If the SSO is clearly a collection system problem or is
unknown, the personnel in maintenance receiving the call
will immediately dispatch an available sewer jet or vac truck
to the call location. The initial personnel and equipment
dispatched may be adjusted based upon the information
received from the initial caller.
11.1.1.3 If the SSO is clearly attributable to a problem at a sewer
pump or lift station, the call will be relayed to the ORC or
back-up ORC.
