On June 8, 2021, WSRO staff reviewed the additional information that was provided on June 2, 2021, related to the 2020
Annual Report. No site visits were made for this inspection event. This additional information provided all the missing items
required for a complete 2020 Annual Report. Info provided reflects compliance with the subject permit. Below is a summary
of follow-up notes on items that were missing in the 2020 Annual Report.
1. Annual Pathogen & Vector Attraction Reduction form (PVRF) were missing for three source facilities with land application
reported in 2020-
i Complete PVRF forms were provided for all three source facilities.
1 Fecal Coliform data provided for all three sources found to be compliant.
1 VAR method provided for all three sources.
1 Annual certification and signatures completed properly.
2. Option 3 (30-Day Bench Scale Test) results and lab info were missing for three source facilities with land application
reported in 2020-
