Enclosed are five copies of an After The fact Pre - Construction Notification (PCN) Permit Application
Package for the City of Raleigh Corwin Road Sanitary Sewer Aerial Maintenance project. For your review
we have enclosed five copies of the PCN Application Form, Attachment A — Additional Information, USGS
Quadrangle Map, site photos, bank stabilization detail, and the $240.00 application fee.
The project titled "Corwin Road Sanitary Sewer Aerial Maintenance" consisted of removing debris
upstream and downstream of a sanitary sewer aerial and stabilizing the stream bank in the immediate
vicinity of the aerial.
The construction described in this permit application resulted in approximately 1,700 square feet of Zone
1 buffer impacts and approximately 400 square feet of Zone 2 buffer impacts within a maintained utility
easement and 40 linear feet of stream disturbance.
In addition to submission to NC DWQ, we are transmitting this PCN electronically to David Shaeffer at
the Army Corps of Engineers.

Attachment A —Additional Information
3d. Project Description
A temporary stream diversion was installed so that all work will occur "in the dry." All work took place with access
from the east stream bank. Abandoned concrete piping was removed from the vicinity of the sanitary sewer aerial.
Debris was removed from bank to bank (approximately 15' wide), returning the streambed to its original grade and
matching the grade upstream and downstream of the work area.
The east and west stream banks within the sanitary sewer easement (Approximately 10' upstream and 30'
downstream of the aerial) were stabilized using geotextile and rip rap. All disturbed areas were stabilized and
reseeded immediately upon completion of work. Conventional construction equipment (rubber -tired backhoe and
dump truck) was used for the work. Photos of the existing site conditions and details are attached.
