The first screen shot shows that the payment submitted on 5/22/2021 for NCC213081 was successful, therefore it was
processed on our end on 5/24/2021, and the COC was generated by email.
Today 5/28/2021, the payment came back as account invalid and was not accepted per the second screen shot..
To resolve this issue, please submit a new check for $125: $100 is the fee for the COC already received, and the $25 is
for the returned item fee, payable to NCDEQ (1612 Mail Service Center, Raleigh NC 27699).
Payment must be received by Monday, June 14t" 2021 to avoid any further compliance action.
Per Part IV, Section B, Item 9 of the NCG010000, failure to pay all administering and compliance monitoring fees in
accordance with 15A NCAC 2H .0105(b)(2) may result in the Division taking enforcement action.
