Enclosed is a report for the inspection performed on, June 20, 2006 by Mr. David Livingston.
Where deficiencies are cited in this report, a response is required as well as for all lettered comments
and/or recommendations. Within thirty days of receipt, please supply this office with a written item for
item description of how these deficiencies, comments and/or recommendations were; corrected. If the
deficiencies cited in the enclosed report are not corrected, enforcement actions will be recommended.
For certification maintenance, your laboratory must continue to carry out the requirements set forth in
15A NCAC 2H .0800.
Copies of the checklists completed during the inspection may be requested from this office.

I. INTRODUCTION:
This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800
for the analysis of environmental samples.
II. GENERAL COMMENTS:
The laboratory is spacious and well equipped. All facilities and equipment are well maintained.
Records are wellkept and most data looked good. Some further quality control procedures need to
be implemented.
III. DEFICIENCIES, REQUIREMENTS; COMMENTS AND RECOMMENDATIONS:
RESIDUAL CHLORINE
Note: Plant is UV operatedand just keeping Residual Chlorine as a back up parameter now.
(A) COMMENT: -If the laboratory- does analyze for Residual Chlorine, an annual curve must be -
established before samples can be analyzed..
REQUIREMENT: The laboratory must do a five point annual curve or a three point curve each
analysis day.
(B) COMMENT: After the laboratory establishes the annual 'curve verify the "gel" type standard
against the,annual curve.
REQUIREMENT: Once the annual curve has been establishedanalyze: the "gel" standard to assign
the initial value of the :"gel" standard. Each analysis day .the "gel" standard must be checked and
found to be within +/- 10% of the initial value.
FYI: Since the laboratory is keeping Residual Chlorine as a backup to the UV dechlorination system,
the laboratory will still be responsible for analyzing a Performance -Evaluation (PE) sample each year.
Before analysis of the PE sample the lab must establish an annual curve._

(C) COMMENT: The laboratory is calibrating the pH meter with 3 buffers, (4, 7, and 10) but does not
reread the 7 buffer as a check.
REQUIREMENT: After calibration analyze a check buffer. The laboratory must document that the pH
7 buffer is checked after calibration and is within 0.1 pH units of the true value:
IV. PAPER TRAIL INVESTIGATION:
This consisted of comparing data on DMRs with the values obtained on laboratory bench
worksheets. All data reviewed for the months of February, March and., April 2006 indicated no
problems in proper transfer of data for reporting purposes. However, in April the laboratory failed to
report any.Dissolved Oxygen readings. Revise and resubmit your. April DMR to. the State: Send
in the revised April DMR with yourresponse to this report.
V. CONCLUSIONS:
Correcting the above cited comments by implementingthe requirements and recommendations will
help this lab to produce quality data and. meet certification requirements. Please respond to the
lettered comments.
