Single Family Wastewater Treatment System

Dear Property Permittee and or Property Owner:
NPDES General Permit NCG550000 and Certificate of Coverage (COC) NCG551626 authorize
the discharge of domestic wastewater from your treatment system to receiving waters designated
as an unnamed tributary to South Fork Little River (classified WS-II; HQW; NSW) in the Neuse
River Basin. The authorized discharge is in accordance with the effluent limits and monitoring
requirements established within the General Permit.
Part II, Section B (14) of NPDES General Permit NCG550000 requires the permittee to "pay the
annual administering and compliance monitoring fee within thirty days after being billed by the
Division. Failure to pay the fee in a timely manner in accordance with 15A NCAC
02H .0105(b)(4) may cause this Division to initiate action to revoke the permit."
The annual fee ($60 per year) for the period ending May 31, 2022 is due by June 7, 2021.
Invoices for the periods ending May 31, 2021, May 31, 2020, May 31, 2019, and May 31, 2018,
respectively are overdue as well. These fees have not been paid and are overdue. Payment in
the amount of $300 must be remitted to the Division as indicated on the attached Annual
Permit Fee Invoices.
The wastewater treatment system should be periodically inspected to ensure the treatment
components are always maintained and in good operating order. You are also reminded to

maintain all monitoring data onsite for a minimum of three years from date of sampling and have
available for inspection.
