Attached please find a Stormwater permit Summary Report for the mine located at 520 Old Vanceboro Road in New
Bern.
It is my suggestion that you complete the following form to change "Ownership" of the mine from Mr. Belangia to the
Company name it is operated under and is registered with the NC Secretary of State.
This will keep correspondence from being sent to him.
Facility name or ownership, complete the Name/Ownership Change Form.
If you do not wish to change the "Ownership, please email your phone number and mailing address to be entered in as
the Billing Contact. This will ensure all future invoices will be sent to your attention.
Please note that the billing month for this certificate of coverage is March.
There are no outstanding permit fees.
