The purpose of the evaluation was to verify that the facility is operating in compliance with the conditions and limitations specified in Permit WQ0004059. Staff members of Fortin Contract Service were present during the inspection. Atlantic Station WWTF consists of an extended aeration biological wastewater treatment package plant with two high -rate infiltration fields with rotary distributer systems. In the summer months, the flow can reach approximately 25,000 gpd of the permitted 100,000 gpd. The sludge holding tank was replaced in late 2019 to meet Schedule 1.1 in the current permit. A review of NDMR/NDAR monitoring records from 2019 to the present shows four fecal, two ammonia and one BOD limit exceedances. A review of GW-59 monitoring records show continuing exceedances in Nitrate and TDS in MW-1. The current permit, issued on October 22, 2019, has a list of conditions under Schedule 1.1-5. To date, the following conditions have not been met: * MW-1A has not been installed within 90 days of the effective date of the permit. Therefore, the condition of submitting an updated site map and a Well Construction Record (Form GW-1) have also not been met (Schedule 1.2,3,4). * MW-1 and MW-3 have not been permanently abandoned within 60 days of the effective date of the permit (Schedule 1.5). In addition, the following items were observed during the inspection: * The EQ tank has holes/leaks in the walls so there is little or no flow control to the dual aeration tanks (15A NCAC 02T .07070)). * The rotary distributers do not rotate as designed and effluent was observed flowing from the center. Also, many spray nozzles were not operable (15A NCAC 02T .07070)). * Ponding of the infiltration sites was observed (15A NCAC 02T 0707(c)). * The MWs are not secured with locks (Inspection NOD on 02/01/2019). It appears to be a challenge to operate the plant and meet the permit requirements due to the present condition of the plant and limited funding. The facility is not compliant with Non -Discharge Permit WQ0004059. An Notice of Violation with Intent to Assess will be issued. When debris clogs the holes, the EQ is operational. The clarifier, located on the south side, was not in operation due to the problematic EQ issues. Both rotary distributors no longer rotate as designed. The arms of the distributers were not rotating as designed and many nozzles were not operable. Ponding and vegetation was seen under the areas where the nozzles were spraying effluent. MW-3 has not been permanently abandoned and MW-1A has not been installed to replace MW-1 as reauired in the current permit schedule. Page 7 of 7
