In accordance with your application received March 31, 1988, we are forwarding
herewith Permit No. 4163-R2, dated June 17, 1988, to the Town of Woodland for the
continued operation of the subject non -discharge type wastewater treatment: facili.t:ies.
"This permit shall be effective from the date of issuance until. November 30,

sha..11. void Permit No. 4163, dated December 16, 1976 and amendments to Permit No. 4163,
dated March 13, 1978 and February 22, 1979.
If any parts, requirements, or limitations contained in this permit are
enacceptable to you, you have the right to request an adjudicatory hearing upon.
written .request within 30 days following receipt of this permit. This request must he
.in the form of a. written petition, conforming to Chapter 150B of North Carolina
Gene.r.al. Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this permit
shall be final and binding.

For the discharge of Sewage, Industrial Wastes, or other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of a 0.185 non -discharge wastewater treatment facility consisting
of a dual 300 GPM lift station with standby power and high water alarm, approximately
12,440 linear feet of. 8-inch diameter force main, a stabilization lagoon, a holding
lagoon, a chlorination system, a flow measuring device, a spray irrigation system
with semi -automatic controls with three (3) vertical turbine 740 GPM pumps,
distribution piping, spray guns with wetting radii of approximately 105 feet, an
approximately 53 acre spray field, and field support equipment to serve the Town of
Woodland, pursuant to the application received March 13, 1986, and in conformity
with the project plan, specifications, and other supporting data subsequently filed
and approved by the Department of Natural Resources and Community Development and
considered a part of this permit.
This permit shall be effective from the date of issuance until rescinded, and
shall be subject to the following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit shall become voidable in the event of failure of the soil to
adequately absorb the wastes and may be rescinded unless the wastewater treatment
and disposal facilities are maintained and operated in a manner which will
protect the assigned water quality standards of the surface waters, and prevent
any contamination of the ground waters which will render them unsatisfactory for
normal use.
5. A suitable year round vegetative cover shall be maintained.
6. Irrigation shall not be performed during inclement weather or when the ground is
in a condition that will cause runoff.
7. Dikes shall be constructed so as to prevent surface water runoff from the spray
field.
8. Diversion or bypassing of the untreated wastewater from the treatment facilities
is prohibited.
9. Solids, sludges, or other pollutants removed or resulting from the wastewater
treatment process shall be contained and disposed of in such a manner as to
prevent any contamination of the surface or groundwaters of the State.
10. The issuance of this permit shall not relieve the Town of Woodland of the
responsibility for damages to surface or groundwaters resulting from the
operation of this facility.
11. The spray field application rates shall not exceed 1 inch/acre/week.
12. Monitoring wells shall be constructed and sampling schedules arranged as required
by the Division of Environmental Management. Additional sampling and records
keeping may be required when deemed necessary to assure proper operation of this
facility.
13. The wastewater treatment and disposal facilities shall be effectively maintained
and operated at all times so that there is no discharge to the surface waters,
nor any contamination. of ground waters which will render, them unsatisfactory for
normal use. In the event that the facilities fail to perform satisfactorily,
including the creation of nuisance conditions or failure of the irrigation Area
to adequately absorb the wastewater, the Permittee shall take such .immediate
corrective action as may be required by the Division of Environmental Management.
14. The spray fields shall be lined as needed to maintain a soil pH of 6.5.
15. The Permittee shall employ a certified wastewater treatment plant operator to be
in responsible charge of the wastewater treatment facilities. Suchoperator must
hold a certificate of the grade at least equivalent to the classification
assigned to the wastewater treatment facilities by the Certification Commission.
16. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will he established and an acceptable
sampling reporting schedule shall be followed. This monitoring may include soil
sampling.
17. Adequate records shall be maintained regarding operation of the subject
treatment and disposa]. system. These records should contain, but not be limited
to, the following:
a. date of irrigation
b. volume of wastewater irrigated
c. zone irrigated
d. length of irrigation, and

e. continuous weekly, monthly, and year-to-date hydraulic (inches per acre)
loadings for each zone.
This information shall be submitted on a monthly basis to the Raleigh Regional
Office on or before the loth of the following month.
18. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, and if warranted, will
extend the Permit for such period of time and ender such conditions and
limitations as it may deem appropriate.
19. An annual soil test shall be performed by a qualified soil scientist and
submitted to the Raleigh Regional Office to ensure that the recommended
agricultural practices are being followed.
20. The existing wells must be sampled every March, July, and November for
the following parameters:

* The measurement of water level must be made prior to sampling for the remaining
parameters.
The results of the sampling and analysis shall be sent to the N.C. Division of
Environmental Management on Form C;W-a"} (Compliance Monitoring Repn1t Form) every
April, August, and December.
