As a result of that inspection, I am sending you this letter to indicate your compliance status and todetail_ the requirements of condition No. 12 ofyour present permit. At the time of the inspection you were found to be in noncompliance with your permit. The following items should be done to attain compliance. 1. All groundwater monitoring wells must be monitored•on a quarterly basis beginning April 1, 1983 for the following parameters: Water level . Total Phosphorus Nitrates BOD Chlorides COD pH 2. Rainfall .should be recorded and reported. 3. Annual soil samples should be initiated to include pH, CEC and nutrient content. The results of this sampling must be reported to this office. 4. The hydraulic and '.nutrient loading should be recorded at the plant for each zone irrigated. Although'this data does not have to be submitted to this office, it should be available upon inspection of the facility. 5. An..aggressive: program of 'sewer rehabilitation should be started to•reduce infiltration and inflow. The continual overloading of the lagoons and subsequeht•spraying during inclement.weather results in a discharge to the surface waters and will damage the integrity of the land application site. w rr, • [Mayor St- n1 oy . 2 March 4, 1963 6. Wastewater Treatment Facilities Northampton County In accordance with your application received June 22, 1982, we are forwarding herewith Permit No. 4163-R, dated October 25, 1982, to the Town of Woodland for the continued operation of <rh_subject non -discharge type vastevtreatment facilities. This permit shall be effective from the date of issuance until November 30, 1987, shall be subject to the conditions and Limitations as specified therein, and shall void Permit No. 4163, dated December 16, 1976 andarendaents to Permit No. 4163, dated March 13, 1978 and February 22, 1979: If any parts, requirements, or limitations contained in this pewit are unacceptable to you, you have the right to an adjudicatory hearing before s hearing officer upon written demand to the Director within thirty (30) days following receipt of this permit, .identifying the specific issues to be contended. Unless such demand is made, this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded to you. Other Wastes In accordance with the provisions of Article 21 of Chapter 143,. General Statutes of North Carolina as amended, and other applicable Lams. Rules, and Regulations PERMISSION 18 HEREBY CRANTID.TO Town of Woodland Northampton County TOR THE continued operation of a 0.185 ion -discharge wastewater treatment facility consisting of a dual 300 0PH lift station with standby poster and high water alarm, approximately 12,440 lineal feat of 8-inch diameter force main, a stabilisation lagoon, a holding lagoon,. a chlorination system, a flow measuring device, a spray irrigation system with semi -automatic' controls with three (3) vertical turbine 740 GPM pimps, distribution piping, spray guns with vatting radii of approximately 105 feet, an approximately 53 acre spray, field, and field support equipment to serve the Town of Woodland, pursuant to the application received .lure 22. 1982, and in conformity with the project plans, specifications and other supporting data. subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this Permit. This Permit shall be effective from the date of issuance until Novs ber.30, 1982, shall be subject to the following specified conditions and limitations, and shall void Permit No. 4163. dated December 16. 1976. and wants to Permit No. 4163, dated March 13. 1978. and February 22, 1979s 1. This permit shall bosoms voidable unless the facilities lure constructed in accordance with the approved plans. spscificseions and other supporting data. . 2. This sit is effective only with respect to the nature and volume of wastes•described in the application and other supporting data. • • • Permit No. 4163-R Page Two 3. The facilities shall be properly maintained and operated at all times. 4. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes, and may be rescinded unless the wastewater treatment and disposal facilities are maintained and operated in a manner which will protect the assigned water quality standards of the surface waters, and prevent any contamination of the ground waters which will render them unsatisfactory for normal use. 5. A suitable year round vegetative cover shall be maintained. 6. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. 7. Dikes shall be constructed so as to prevent surface water runoff from the spray field. 8. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. 9. Solids, sludges, or other pollutants removed or resulting from the wastewater treatment process shall be contained and disposed of in such a manner as to prevent any contamination of the surface or ground waters of the State. 10. The issuance of this permit shall not relieve the Town of Woodland of the responsibility for damages to surface or ground waters resulting from the operation of this facility. 11. The spray field application rate shall not exceed 1 inch/acre/week. 12. Monitoring wells shall be constructed and sampling schedules arranged as required by the Division of Environmental Management. Additional sampling and records keeping may be required when deemed necessary to assure proper operation of this facility. 13. The wastewater treatment and disposal facilities shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters which will render them unsatisfactory for normal use. In event the facilities fail to perform satisfactorily, including the creation of nuisance conditions, or failure of the irrigation area to adequately absorb the wastewater, the Permittee, shall take such immediate corrective action as may be required by the Division of Environmental Management. Permit No. 4163-R . Page Three 14. The spray fields shall be limed as sneeded to maintain a soil pH of 6.5. • 15. The Pernittee shall employ a certified wastewater treatment plant operator in responsible charge of the wastewater treatment facilities. Such operator must hold a certificate of the grade at least equivalent to the classification assigned to the wastewater treatment faeilitiee by the Certification Commission. 16. Any monitoring deemed necessary by the Division of Environmental Management to insure surface and ground water protection will be established and en acceptable sampling reporting schedule shall be followed. This motoring may include soil sampling. 17. Reports on the operations of the Tow of Woodlands Wastewater Treatment System shall be submitted to the Raleigh Regional Office at regular intervals and in such form and detail as may be required by this Division. 18. The Psrmtttee, at least six (6) months prior to the expiration of this Permit. shall request its extension. Upon receipt of the request, the Couiseion will review the adequacy of the facilities described therein, and if indicated, will extend the Permit for such period of time and under such conditions and limitations as it may deem Appropriate.
