Division of Water Resources' Central and Regional staff has reviewed the application package
received March 24, 2021. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on June 24, 2021.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through F, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with one

1. The table in Section V (ORC Information) must be completed. Please designate a Back -Up ORC.
D. Site Map:
1. This site was originally permitted and had a major modification after September 1, 2006; therefore,
a site map is required. The map submitted does not meet the minimum requirements. Please submit
a site map meeting the requirements of 15A NCAC 02T .0704(d).
At a minimum, the map should include the following information:
a. Boundaries and physical features not under purview of other licensed professions shall be provided by a
professional land surveyor
b. Scale no greater than 1-inch equals 100 feet (special provisions may be granted upon prior approval for
large properties)
c. Legend, north arrow, scale, and legible in black and white
d. Topographic contour intervals not exceeding two feet
e. All facility -related structures and fences within the treatment, storage, and disposal areas
f. All habitable residences and places of assembly within 500 feet of all treatment, storage, and irrigations
sites
g. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds, ditches, and
other surface drainage features within 500 feet of all waste treatment, storage, and disposal site(s)
h. Location and identification of each monitoring well (identify any background/upgradient wells)
i. Latitude and longitude coordinates of all monitoring wells (decimal degrees to 6 digits and in NAD83)
j. Location and identification of major components of the waste disposal system
k. The wetted perimeter of all irrigation fields with field names (named according to the approved permit)
1. Location and ownership of property boundaries within 500 feet of the disposal area (including road/rail
right-of-way and easements)
m. The delineation of compliance and review boundaries
n. Distance measurements verifying all setbacks are being met
o. Stormwater drainage controls, and
p. 100-year floodplain.

E. Operation & Maintenance Plan:
1. A copy of the Operation and Maintenance Plan has not yet been provided to the Division. Please
submit a copy of the Operation and Maintenance Plan, pursuant to 02T .0707(a).
This plan should include at a minimum•
i. description of operation of the system in sufficient detail to show what operations are necessary for
the system to function and by whom the functions are to be conducted;
ii. anticipated maintenance of the system;
iii. safety measures including restriction of access to the site and equipment, as appropriate; and
iv. include spill control provisions including:
a) response to upsets and bypasses including control, containment, and remediation; and
b) contact information for plant personnel, emergency responders, and regulatory agencies.
F. Additional Documentation:
➢ Operational Agreements:
1. Please submit a fully executed and signed copy of Form HOA, the Operational Agreement
form. A blank copy has been provided for your convenience.
➢ Certificate of Public Convenience and Necessity:
1. If the Riversound WWTP has been authorized as a privately -owned utility by the North
Carolina Utilities Commission, please submit a copy of the Certificate of Public Convenience
and Necessity. An Attachment D was not included with the application package.
