On June 14, 2007, the Wetlands/401 Unit of the Division of Water Quality received a Fax/email request
from George Buchholz of the John R. McAdams Company, Inc., regarding a project known as "Highland
Creek Subdivision Phase 2 & 4 stormwater management plan" for acceptance into the Express Review
Program.
This letter advises you that your project will be accepted into the Express Review Program on or after
June 25, 2007: '
a. The application fee of $1500.00 made payable to the North Carolina Division of Water
Quality;
b. Two (2) complete and collated copies of the Express Review Stormwater Management Plan

c. Two (2) copies of all supporting information pertaining to this project (site development
plans, stormwater management plans, worksheets, etc.);
d. One (1) original and one (1) copy of an Operation & Maintenance Agreement for the
stormwater management facilities.

Please explicitly mark all correspondence and transmittal sheets as "EXPRESS REVIEW" to
ensure delivery to the Express Review Staff.
