Please note that I am not requesting written authorization for the Mims Stormwater Control Structure project, as the
project does not meet the conditions for requiring written authorization. There was an error in the application which
specified that I was asking for written approval. I am simply providing a courtesy copy of the application to DWQ.

Looking through what was received yesterday, I need 2 more PCN sets for a total of 5, plus according to the GC3890 a
permitting fee of $240 will need to be remitted to begin the review & approval process because you are seeking written
approval. We also did receive the revised impact maps yesterday afternoon.
I don't really want to send the whole package back so how quickly do you think you could get me what is needed?
