This office has received a copy of Mining Permit Number 83 -06. The pit is approved for any
borrow to be used on the project referenced above. The determination of whether the material is
suitable will be in accordance with Article 230 of the 2012 North Carolina Department of
Transportation Standard Specifications for Roads and Structures. In addition please reference
Page 31 in the Contract entitled Shoulder and Fill Slope Material.
Please note that, in addition to the mining permit requirements, any applicable state and local
ordinances shall be complied with. Attached, please find a copy of the mining permit cover and
site map.

Your application for renewal of the above referenced mining permit has been approved.
A copy of the renewed permit is enclosed. The new expiration date is April 15, 2019,
The conditions in the permit renewal were based primarily upon the initial application.
Modifications were made as indicated by the renewal request and as required to insure
compliance with The Mining Act of 1971. 1 would like to draw your particular attention to the
following conditions where minor additions or changes were made: Operating Condition Nos.
1 B, 3A, 413, and 4C and Reclamation Condition No. 3.
As a reminder, your permitted acreage at this site is 10.66 acres and the amount of land
you are approved to disturb is 7.43 acres.
