Division of Water Resources' Central and Regional staff has reviewed the application package
received February 11, 2021. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on May 6,
2021.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through C, or failure to provide the additional information on or before the above requested

A. It was noted that Monitoring Wells MW-7, MW-8, MW-9, and MW-10 are associated with the golf
course, which are proposed to be removed in this minor modification. Please let us know whether there
is a plan to use these monitoring wells in the future, which would help us decide whether these wells
should be temporarily abandoned by securing the wellheads to prevent access or be permanently
abandoned by a certified well contractor.
B. Based on the March 18, 2021 site visit, the operators list is not up to date, see attached affiliation list.
Pursuant to 15A NCAC 08G .0201, Please contact Beth Buffington with our Operator Certification
Program at [e-mail] to update the list with updated contact information.
C. According to our records, Mr. Dana Hill is not one of the designated signature authorities yet. Pursuant
to 15A NCAC 02T .0106, please have the signing official sign the application or have the signing
official complete the attached signature authority designation letter. Please also let us know if any of
the contacts in the attached affiliation list is not up to date.
