EASLEY LYI~T,,IPPETT GOVERNOR SECR~ RY April 23, 2007 Mr. David Baker, NCDOT Regulatory Project Manager ~0~ US Army Corps of Engineers U 151 Patton Avenue, Room 208 Asheville, NC 28801-5006 Subject: 404 and 401 Permit Application Revision/Modification Action ID No. 2006-32917-345 DW roject No. 060987 SR 1836 -Pot Shoals and Camp Creek Roads -Henderson County State Project No. 140.045131 (DWQ Minor Permit Fee $200.00) Dear Mr. Baker: The North Carolina Department of Transportation (NCDOT) proposes to widen, grade, drain and pave SR 1836 in Henderson County. The original permit applications dated June 19, 2006 included twenty-two permitted sites with pipe extensions totaling 302 linear feet. Due to a grade change at Site #10, Station 61 +70, the original length was inadequate. NCDOT proposes to install an additional 20 linear feet of 24-inch CMP to gain enough length to stabilize the fill slope. I have enclosed a revised PCN application (impact table), topographical map and cross-sectional drawing for the culvert installation (Site 10). The new culvert will remain perched, as is the existing culvert. Impacts to Waters of the United States In order to construct the project in accordance with our current secondary road standards, it will be necessary to impact waters of the United States in the Broad River Basin. Specifically, NCDOT is requesting to add 20 linear feet of culvert in an unnamed tributary to Green River (DWQ Class: B, Tr). Listed below is a revised summary of the proposed impacts. u.2 Division 14, District 1 Phone: (828)891-7911 4142 Haywood Road, Mills River, NC 28759 Fax: (828)891-5026 r Site No. Station Existing Condition Proposed Condition Net Impacts Site 1 - 13+80 45' x 30" CMP 60' x 36" CMP w/gabions 15' Site 2 -16+15 35' x 36" CMP 55' x 48" CMP w/gabions 20' Site 3 - 21+90 30' x 42" CMP 50' x 48" CMP w/gabions and rock buttress 20' Site 4 - 23+90 35' x 18" CMP 55' x 30" CMP w/ gabions 20' Site 5 - 29+52 35' x 30" CMP 55' x 36" CMP w/gabions 20' Site 6 - 46+74 30' x 18" CMP 50' x 18" CMP 20' Site 7 - 52+70 New Alignment -none existing 120' x 72" CMP w/ headwalls 120' Site 8 - 53+30 30' x 18" CMP Remove -flows to Site 7 -30' Site 9 - 54+75 35' x 48" CMP Remove -flows to Site 7 -35' Site 10 - 61+70 50' x 18" CMP 70' x 24" CMP 20' Site 11 - 64+35 35'x18"CMP& 55' x 18" Terra Cotta w/ rock headwall 80' x 36" CMP 45' Site 12 - 68+72 40' x 18" CMP 70' x 24" CMP 30' Site 13 - 73+04 35' x 18" CMP 50' x 18" CMP 15' Site 14 - 79+24 50' x 18" CMP 60' x 24" CMP 10' Site 15 - 111+42 35' x 24" CMP 50' x 36" CMP 15' Site 16 -115+05 25' x 15" CMP 40' x 18" CMP 15' Site 17 - 119+77 30' x 18" CMP 45' x 24" CMP 15' Site 18 - 161+72 7' H x 15' W RC earth filled Spandrel Arch Remove and replace with abridge -15' Site 19 - 179+06 40' x 18" CMP 50' x 24" CMP 10' Site 20 -190+35 18' x 24" Terra Cotta w/ rock headwalls Remove and replace with abridge -18' Site 21 - 202+49 (2) 50' x 84" x 66" CMP Arch (2) 50' x 87" x 63" CMP Arch 0' Site 22 - 208+03 35' x 42" CMP 45' x 48" CMP 10' Total Impact 322 linear feet permanent Permits Requested Also by copy of this letter, I am requesting authorization to proceed under Section 401 of the Clean Water Act from the Division of Water Quality. If a stream has no name, list as UT (unnamed tributary) to the nearest downstream named stream into which it flows. USGS maps are available through the USGS at 1-800-358-9616, or online at www.us~s.QOV. Several Internet sites also allow direct download and printing of USGS maps (e.g., www.topozone.a>m, www.m~guest.com, etc.).
