1) The drawings were revised to delete the stub road as requested (see attached revised drawings). This will
reduce the permanent wetland impact from 0.074 acres to 0.029 acres and eliminate 0.018 acres temporary
wetland impact. ( see attached revised impact table).
I also requested the Engineer to talk with the developer about any other earlier impacts (I noticed culverts
were installed on Ray Drive and Will Drive in the past). I had the engineer check with the developer, he said
the earlier phases were installed he thinks in the 1980's when he was in high school.They have no record of
any permits. I checked with Google Earth, the culverts were installed prior to 1993. Thats as far as I could go
back.

GENERAL NOTES FOR THIS PLAN SHEET:
1. This plan sheet is part of multiple plan sheets within an overall set of Confructlon Drawings for Site Conttuctian only, The plan sheets in the
construction drawings are designed, engineered, and produced In accordance with 21 NCAC 56.0701 and.1103, Each plan sheet within
the Construction Drawings shall not be considered final for construction unless appropriately signed and seated by Engineer as a final
document. Additionally, all site permits and approvals from appropriate federal, state 8, local agencies must be obtained prior to
beginning any construction activity related to a permit/approval. Contact Engineer if there is any question regarding permits/approvals.

sheet for additional notes or specifications pertaining to the overall project.
SPECIAL/ADDITIONAL NOTES FOR THIS PLAN SHEET;
I. Purpose: The purpose of this Overall Grading Plan (with Environmental lmpactsL is to depict an overall layout with final contours for most site
construction activities that are related to grading or earth moving. More detailed information pertaining to erosion control and surface drainage
can be found on the Erosion Control Plan, This sheet may also depict construction limits in environmentally sensitive areas and other details
related to any environmental impacts (le. Wetlands, Stream Buffers, etc.), IF ANY. This plan sheet is part of a full set of Construction Drawings and
should be used in conjunction with the full set that is approved and released for construction.
2. Standard Specifications: All grading far street construction or other vehicular areas lie. parking fats) shall be in accordance with the current standards
and specifications of NCDOT. When required or requested, CONTRACTOR shall provide for independent testing & inspection, Including
proof -rolling, compaction, and asphalt density testing; and shall provide written documentation to DEVELOPER/ENGINEER Indicating acceptable
results. Al! other grading shall be in accordance with these Plans & Specifications; or otherwise in accordance with industry standards. Alf Erosion
Control construction shall be in accordance with the current minimum standards and specifications of NCDEQ and all conditions of permitting.
And any construction In Environmentally Sensitive Areas shall be in accordance with all conditions of any related permit. See
Notes/Specs/Legend sheet far additional specifications.
3. Work in Rights -of -Way or Easements: Any work in existing or proposed rights -of -way or easements tparticutarly NCDOT rights-of-wayjshall be conducted
according to the standards of the holder of the right-of-way or easement. Work in existing rights -of -way or easements shall also be conducted in
accordance with any approved encroachment or permit, and all conditions within the the permit. Contact DEVELOPER or ENGINEER with any
questions regarding permitting.
4. Finish Grade Contours: The finish grade contours shown on this plan are to all finish surface elevations (ie. asphalt, sidewalk, curb, gutter, or ground
elevations. ANY OTHER GRADING SHALL BE TO ELEVATIONS AS SPECIFIED, OR OTHERWISE FOR POSITIVE
DRAINAGE.
5. Handicap Accessibility: This site shall conform to all requirements for handicap accessibility. This may require additional asphalt or concrete surfaces as
may be required by local inspector. It may also require that special attention be given to finish grades throughout ALL stages of grading. In
general, proposed grades and contours as indicated hereon were set in order to meet the required standards. However, because of the detail
In construction required to achieve these requirements in the field, the CONTRACTOR shaft be responsible for ensuring that staking, rough
grading, base material installation, fine grading, and finish surface result in the desired finish grades & slopes along accessible routes. See
Special Notes - Handicap Accessibility on Sheet I for additional information related to Handicap Accessibility.

Twin Oaks is not a large development, but it has been going on since at least 2001 and has been expanding slowjy and in small segments. As I noted above this phase is only 18 lots.
To best of my knowledge this should be the last expansion.
Are any other NWP(s), regional general permlt(s), or individual permits(s) used, or intended to be used, to authorize any part of the proposed project or related activity? This
includes other separate and distant crossing for linear projects that require Department of the Army authorization but don't require pre -construction notification.
Best of my knowledge this is the last phase and no other is planned. the sanitary sewer was completed many years ago.

The stub road and stormwater basin was designed to minimize Impact. The Impact could not be avoided since the stub outs on north and south of
Peyton Drive were required by the TOW' of Smithfield.

Care will be taken not to drive in or impact the wetlands outside the permitted area. adequate sediment control will be installed to intercept sediment
