Notice of Deficiency Response
Dear Mr. Bell,
We are in receipt of a copy of the Notice of Deficiency regarding your findings during your
April 13, 2021 inspection of the above system. This copy was provided to us by Mr. Bradshaw.
We also are in receipt of a copy of the permit for the above system provided to us on April 20,
2021 via email from you.
The plan of action to address the noted violations is to sample the parameters listed on the
permit provided to us going forward in 2021. The homeowner will be provided copies of those
results. In the event any results are outside of the acceptable range listed, the system will be re-
evaluated and any parameters outside of range will be resampled to be sure they are compliant.
