However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on June 17, 2021. In order to continue using the full 23.4 acre application acreage as requested, setback waivers are needed to allow reduced setbacks. 2. Setback waivers allowing reduced setbacks from Field CH-01 to habitable residences are required for the following parcels: a. Setback distances have changed since the last permit issuance (e.g. setbacks to surface water have been reduced to 32.8 feet). Sullins Dee Freeman Governor Director Secretary Map Guidance for Residuals Land Application Fields (Non -dedicated) June 9, 2011 Minimum map submittal requirements for residuals land application permits are specified in 15A NCAC 02T .1104. The following guidance is intended to provide clarification of Division expectations for submitted maps in order to assist Applicants in preparing complete submittals. All maps are to be originals or 1St generation copies to insure they are clear and legible, and should be labeled with the following information: • Scale (appropriate scale shall be used to cover the objective) • County where site is located • Landowner's name • Name of the Permittee and/or permit number • Site identification number (if applicable) • Map legend (if applicable) The 15A NCAC 02T .1104 application requirements do not specify how many maps to submit or what required information must be submitted on each map. The Division recommends the Permittee divide the required information onto each of the following maps. 1. Vicinity map (road map): • Field location(s) for the site to be permitted • At least two geographic references (e.g., numbered roads, named streams/rivers) 2. Topographic Map: • Field location(s) • Topographic contour intervals not exceeding 10 feet or 25 percent of total site relief • Show all facility related structures and fences within the treatment, storage and disposal areas 3. The Application Area & Setback Map may include a note indicating the delineation of the compliance and review boundaries instead of the line drawn on the map [e.g., "The compliance boundary is established at (either 250 feet from the residuals land application area or 50 feet within the property boundary, whichever is closest to the residuals land application area). The review boundary is established midway between the compliance boundary and the perimeter of the residuals land application area."]. • The 15A NCAC 02L rules prohibit water supply wells within the compliance boundary and require wells to be located 250 feet away from the disposal area. An alternative to this requirement can be approved in accordance with 15A NCAC 02T .0105(h). Upon the Permittee's request, the compliance boundary may be located closer to the waste disposal area due to an on -site water supply well. Maps showing wells with the re-established compliance boundary must delineate or include a footnote describing the redefined compliance and review boundaries (e.g. "A re -defined compliance boundary is proposed for this land application site. This re -defined compliance boundary is south of the field and shall be located at 100 feet from the water supply well on Harold Eldridge's property"). 5. Critical Watershed Map: • Clearly show that the proposed fields are not located within critical watershed areas. Include a critical watershed area map for each county where the proposed land application sites are located. • Send a copy of this map to the County Manger along with the County Manager Notification Form (CMNA 08-09) prior to application submittal. • Water Supply Watersheds Critical Areas maps may be downloaded at: http://portal.ncdenr.org/web/wq/swp/ws/wswp/maps 6. Aerial Map (if applicable): • An aerial reference map may be included and is desirable if during a site review the field to be permitted has been planted in a row crop and cannot be easily evaluated.
