Centigrade (00010) 11/24/2018 5 X week Frequency Violation Action Taken: The week of November 18, 2018, was the week of Thanksgiving. The reading would normally have been done on Thursday, November 22, 2018, which was Thanksgiving Day. The fifth required reading for the week should have been done on November 24, 2018, which was missed. The ORC was unavailable to perform the reading on November 24th, and the individual who would normally perform the reading in the absence of the ORC was inadvertently not notified that the reading needed to be performed. This situation has been brought to the attention of all parties, so that in the future when assistance is needed in order to perform the required readings the proper notice is provided to all of the appropriate individuals. I ask that you take the above responses into consideration when reviewing the information and circumstances surrounding the noted violations and the determination of the civil penalty to be assessed.
