Does it have a "Parent" owner entity, or is it owned by a partnership or by an individual doing business
as OK Recycling?

Complete a Name/Ownership Change Form. Changing ownership from Bob Miller to SDLogistics.
If you run an Owner Verification Report, in Step 1, it will give you some of the information
needed for the form.

Hi Suzanne, I am taking over OK Sales NPDES industrial stormwater reporting responsibility from Bob
Miller who is no longer directly employed by the company. I am trying to verify and register
electronically and submit the E reporting form. Steps 1 and 2 of 6 total. I am hoping to be assigned to
the permit as the responsible official as I will be taking on the responsibility from here forward of facility
data maintenance and filings. What is the normal protocol in this situation, is this a simple matter of
updating the information on the permit, or do we need to file a whole new NPDES permit?
